The significance of the histochemical evaluation of nucleolar organizer regions (AgNORs) and of the immunohistochemical expression of proliferating cell nuclear antigen (PCNA), and immunohistochemical expression of the oncogenes c-myc and Ha-ras, in multiple myeloma: evaluation in 180 bone marrow biopsies

We have studied 180 cases of Multiple Myeloma, 176 of which concerned bone marrow trephine biopsies and 4 solitary extramedullar plasmacytomas. Our material, was formalin-fixed, paraffin-embedded tissue sections of 4ηιμ thick which were initially studied by using conventional histochemical methods and further on with the standard immunoperoxidase methods of peroxidase-antiperoxidase, avidin-biotin-peroxidase complex (ABC) and Streptavidin-biotin-peroxidase complex (Strepi ABC). We have also performed the colloidal silver staining method for the demon stration of the argyrophilic nucleolar organizer regions (AgNORs). The antibodies used to carry out the above mentioned methods were the following: 1. Polyclonal antibodies against the immunoglobulin κ and λ light chains. 2. Polyclonal antibodies against the immunoglobulin heavy chains: IgG, IgA, IgM, IgD, IgE. 3. Monoclonal antibody L-26 which recognizes the homonym antigenic determinant. 4. Monoclonal antibody 1-9E10 which identifies the p62 protein, encoded by the c-myc oncogene. 5. Monoclonal antibody Y13 259 which identifies the p21 protein encoded by the Ha-ras oncogene. 6. Monoclonal antibody PC-10 against the Proliferating Cell Nuclear Antig en/Cyclin. The aim of our study was to investigate the following: a) the expression of the oncogenes c-myc and Ha-ras in Multiple Myeloma (M.M.), b) the evaluation of the utility of the monoclonal antibody PC-10 in the determination of the biological behaviour of this malignant neoplasm, c) the evaluation of the AgNORs count in the histological grading of M.M. d) the specificity of the monoclonal antibody L-26 in differentiation of M.M. from other Β cell lymphoproliferative disorders and finally e) the necessity of immunohistochemical methods to evaluate the immunophenotype of malignant plasmacytes in M.M. The results concerning the above antibodies expression in M.M. are summarized as follows: 1. L-26 Monoclonal antibody is a very specific and sensitive marker for B-cell malignancies except for Multiple Myeloma which is lacking this antigen. Therefore it might be helpful in the differential diagnosis of M.M. from other B-cell malignancies. 2. The IgG immunophenotype is the commonest one met in Multiple Myeloma. 3. The evaluation of the morphology of the neoplastic plasma cells as a unique criterion for the immunophenotype of M.M. should be avoided and immunohistochemistry must always be performed for the identification of the M-protein produced by malignant plasmacytes. 4. The number of AgNORs although considered as a good prognostic marker must not be evaluated as such, but in association with other prognostic clinical and/or laboratory findings and markers. 5. The PC-10 monoclonal antibody, is a particularly reliable and specific marker in the evaluation of proliferation status of M.M. cells and hence of its biological behaviour. 6. The immunohistochemical expression of c-myc oncogene, correlates strongly with these of the PC-10 and the AgNORs count and hence, with the proliferation status of M.M. It is noteworthy that the c-myc Pgg protein, expressed in malignant plasmacytes, does not show any relevance with their morphology and infiltration patterns of bone marrow. 7. The expression of Ha-ras oncogene at the immunohistochemical level, is probably related to the pathogenesis of M.M.(its correlation with c-myc being statistically indicative). Its quantitative level does not appear to correlate with the morphology of plasmacytes and/or infiltration patterns and hence with the histological grade of malignancy of M.M. However, the relationship between its expression level and the biological behaviour of this neoplasm, might not be excluded independently of the histological grade or other prognostic markers; this is a matter of further investigation for the molecular biologists and immuno-cytogenetics.Μελετήσαμε 180 περιπτώσεις πολλαπλού μυελώματος (Π.Μ.). Εξ αυτών οι 176 αφορούσαν σε οστεομυελικές βιοψίες και 4 σε μονήρη εξωμυελικά πλασματοκυττώματα. Το υλικό μας αφορούσε σε τομές παραφίνης, πάχους 4μιτι στις οποίες αρχικώς μεν εφαρμόσθηκαν συμβατικές ιστοχημικές χρώσεις, στη συνέχεια δε, ανοσοϊστοχημικές μέθοδοι υπεροξειδάσης-αντιϋπεροξειδάσης (PAP) και αβιδίνης- βιοτίνης/οτρεπταβιδίνης-βιοτίνης (ABC/Strept-ABC) καθώς και η ειδική χρώση κολλοειδούς νιτρικού αργύρου, για την κατάδειξη των περιοχών οργανώσεως του πυρηνίου. Τα αντισώματα που χρησιμοποιήθηκαν κατά την εφαρμογή των ως άνω μεθόδων είναι τα εξής: 1. Πολυκλωνικά αντισώματα αντί-κ αντί-λ ελαφρών αλύσεων. 2. Πολυκλωνικά αντισώματα αντί-lgG, αντί-lgA, αντί IgM, αντί IgD, avri-lgE βαρειών αλύσεων. 3. Μονοκλωνικά αντισώματα avri-L-26 που αναγνωρίζει τον ομώνυμο αντιγονικό επϊτοπο. 4. Μονοκλωνικό αντίσωμα 1-9Ε10 για το ογκογονϊδιο c-myc. 5. Μονοκλωνικό αντίσωμα Υ13 259 για το ογκονΐδιο Ha-ras. 6. Μονοκλωνικό αντίσωμα PC-10 κατά του Πυρηνικού Αντιγόνου πολλαπλασιαζόμενων Κυττάρων/κυκλίνης. Σκοπός της εργασίας μας ήταν α) η μελέτη της εκφράσεως των ογκογονιδίων c-myc και Ha-ras στο Π.Μ. β) η εκτίμηση της χρησιμότητος του μονοκλωνικού αντισώματος PC-10 στον καθορισμό της βιολογικής συμπεριφοράς του νεοπλάσματος αυτού γ) η εκτίμηση της χρησιμότητος της εκφράσεως των περιοχών οργανώσεως του πυρηνίου στην ιστολογική ταξινόμηση του Π.Μ. δ) η αξιοπιστία της χρησιμότητος του μονοκλωνικού αντισώματος L-26 στη διαφορική διάγνωση του Πολλαπλού Μυελώματος από άλλα Β-λεμφοϋπερπλαστικά νοσήματα, και ε) η απαραίτητη εφαρμογή ανοσοϊστοχημικών μεθόδων στην ταυτοποίηση του ανοσοφαινοτύπου των κακοηθών πλασματοκυττά- ρων στο Π.Μ. σε ιστολογικό επίπεδο. Σύμφωνα με τα αποτελέσματα μας, τεκμαίρονται τα κατωτέρω: 1. Ο IgG ανοσοφαινότυπος είναι ο συχνότερον απαντώμενος στο Πολλαπλούν Μυέλωμα. 2. Μόνη η μορφολογία δεν αρκεί για τον καθορισμό του ανοσοφαινοτύπου του Π.Μ. και χρειάζεται εφαρμογή ανοσοϊστοχημείας για την ταυτοποίηση του. 3. To L-26 είναι ευαίσθητος και πολύ αξιόπιστος δείκτης στη βοήθεια διαφορικής διαγνώσεως πολλαπλού μυελώματος από άλλα Β-λεμφοϋπερπλαστικά κυρίως νοσήματα. 4. Οι περιοχές οργανώσεως του πυρηνίου (AgNORs) αποτελούν δείκτη προγνώσεως, στο μέτρο όπου υφίσταται ταυτοχρόνως συνεκτίμηση και των λοιπών κλινικοεργαστηριακών ευρημάτων. 5. To PC-10, είναι ιδιαίτερα αξιόπιστος δείκτης αυξητικού δυναμικού του Πολλαπλού Μυελώματος και θα μπορούσε να θεωρηθεί δείκτης βιολογικής συμπεριφοράς του νεοπλάσματος αυτού. 6. Το ογκογονίδιο c-myc συνδέεται με το βαθμό κακοηθείας του πολλαπλού μυελώματος, σχετιζόμενο με την αυξητική τάση των πλασματοκυττάρων (PC-10 και AgNORs). Δεν φαίνεται να συνδέεται στενά με τη μορφολογία και το πρότυπο διηθήσεως του μυελού. 7. Το ογκογονίδιο Ha-ras, ενέχεται ενδεχομένως στην απιοπαθογένεια του πολλαπλού μυελώματος (συνδέεται έστω και ενδεικτικώς με το c-myc). Η ποσοτική του έκφραση δεν φαίνεται να σχετίζεται με τη μορφολογία των πλασματοκυττάρων, με τον τρόπο διηθήσεως κι επομένως με τον ιστολογικό βαθμό κακοηθείας του Π.Μ. Ομως η σχέση επιπέδου εκφράσεως του με τη βιολογική συμπεριφορά αυτού του νεοπλάσματος, δεν θα έπρεπε ν'αποκλεισθεί, ανεξαρτήτως του ιστολογικού βαθμού κακοηθείας ή άλλων προγνωσπκών δεικτών.
