Endometrioma de la pared abdominal como diagnóstico diferencial poco frecuente de un tumor de tejidos blandos

Introduction: Endometriosis is characterized by the location of normal endometrial tissue, glands, and stroma,outside the uterine cavity. Its frequency ranges from 3 to 15% of women of childbearing age. The mostcommon location is intra-abdominal, but it can also be extra-pelvic. Extra-pelvic endometriosis is infrequentand is classified, according to its location, into four types: gastrointestinal, urinary tract, thoracic and "othersites", in the form of painful nodules on the abdominal wall and scars from previous surgical interventions,mainly gynecological or obstetric or as inguinal or umbilical nodules. The prevalence of localizedendometriosis in the abdominal scar after surgery ranges from 0.1 to 0.4%. Objective: Describe a clinical case of endometriosis infrequently located in the abdominal wall. Material and methods: Descriptive, retrospective study, presentation of a clinical case at the "IESS LatacungaBasic Hospital", a case that was treated in the General Surgery service of the hospital. Results: Description of the clinical case: a 68-year-old female patient with a personal clinical history of type 2diabetes mellitus, non-insulin dependent, being controlled with oral hypoglycemic agents; high blood pressurediagnosed 30 years ago in treatment; allergies: does not refer; surgical history: pelviperitonitis due to abscessof the left ovarian tube, umbilical hernia and ectopic pregnancy. Current disease: Patient reported thatapproximately 5 months ago he presented a mass in the umbilical region, of progressive growth, painful onphysical exertion, of low to moderate intensity without other accompanying symptoms, which is why he wentto the doctor, who decided to perform an echo of soft parts where reported: in the mesogastrium regionanechoic image of defined edges with echogenic mural and other central areas, avascular to the Doppler,measures 13.06 x 9.78 x 7.56 cm; Due to its size, it is not possible to identify a dependency body. Reason forwhich she went to the IESS Latacunga Hospital where she was admitted to the General Surgery Service, asimple and contrasted Computerized Axial Tomography of the Abdomen and pelvis was performed, the samethat was reported: in the anterior abdominal wall at the level of the mesogastrium and hypogastrium in themidline, in the thickness of the deep portion of the subcutaneous cellular tissue, hypodense, liquid-densityimage measuring approximately 14 cm in diameter and having a volume of approx. 690 ml. Tumor markerswere requested: embryonal carcinogen antigen (CEA): 1.58 ng / ml, CA-125: 9 U / ml, CA19-9: 9.10 U / ml,AFP alphafetoprotein: 0.67 ng / ml. After presenting the case to all the Staff of the General Surgery Service,its surgical resection is decided. Conclusions: Considering that the incidence of endometriomas of the abdominal wall is low, before any nodule with abnormal characteristics in the inguinal or umbilical region, or a tumor on a gynecological or obstetric scar in a woman of fertile age, it is important to consider and suspect the presence of anendometrioma. La endometriosis extrape?lvica es infrecuente y se clasifica, según su localizacio?n, en cuatro tipos: gastrointestinal, del tracto urinario,torácica y de “otros sitios”, en forma de no?dulos dolorosos en la pared abdominal y en cicatrices deintervenciones quirúrgicas previas, fundamentalmente ginecolo?gicas u obste?tricas o como no?dulos inguinaleso umbilicales. La prevalencia de endometriosis localizada en la cicatriz abdominal despue?s de una cirugíaoscila entre 0,1 y 0,4%. Objetivo: Describir un caso clínico sobre endometriosis de ubicacio?n infrecuente en la pared abdominal. Material y me?todos: Estudio descriptivo, retrospectivo, presentacio?n de caso clínico en el “Hospital BásicoIESS Latacunga”, caso que fue tratado en el servicio de Cirugía General del hospital. Resultados: : Descripcio?n del caso clínico: paciente femenina de 68 años de edad, con antecedentes clínicospersonales de diabetes mellitus tipo 2, no insulino dependiente, siendo controlada con hipoglicemiantesorales; hipertensio?n arterial diagnosticada hace 30 años en tratamiento; alergias: no refiere; antecedentesquirúrgicos: pelviperitonitis por absceso tubo ovárico izquierdo, hernia umbilical y embarazo ecto?pico.Enfermedad actual: Paciente refirio? que hace aproximadamente 5 meses atrás presenta masa en regio?numbilical, de crecimiento progresivo, dolorosa al esfuerzo físico, de baja a moderada intensidad sin otrasintomatología acompañante, razo?n por la cual acudio? a me?dico, quien decidio? realizar un eco de partesblandas donde reporto?: en regio?n de mesogastrio imagen anecoica de bordes definidos con áreas ecoge?nicasmurales y otras centrales, avasculares al doppler, mide 13.06 x 9.78 x 7.56 cm; por su tamaño no se lograidentificar o?rgano de dependencia. Motivo por el cual acudio? al Hospital IESS Latacunga donde es ingresadaal Servicio de Cirugía General, se realizo? una Tomografía Axial Computarizada simple y contrastada deAbdomen y pelvis, la misma que reporto?: en la pared abdominal anterior a nivel de mesogastrio e hipogastrioen la línea media, en el espesor de la porcio?n profunda del tejido celular subcutáneo imagen ovaladahipodensa con densidad de líquido que mide aproximadamente 14 cm de diámetro mayor y tiene un volumenaprox. de 690 ml. Se solicito? marcadores tumorales: antígeno carcino embrionario (CEA): 1.58 ng/ml, CA-125: 9 U/ml, CA19-9: 9.10 U/ml, AFP alfafetoproteina: 0.67 ng/ml. Posterior a presentar el caso a todo elStaff del Servicio de Cirugía General se decide su reseccio?n quirúrgica. Conclusiones: Considerando que la incidencia de endometriomas de pared abdominal es baja, ante cualquierno?dulo de características ano?malas en la regio?n inguinal o umbilical, o tumoracio?n sobre cicatriz ginecolo?gicau obste?trica en una mujer de edad fe?rtil, es importante considerar y sospechar la presencia de unendometrioma. Los me?todos de imagen y la puncio?n-aspiracio?n con aguja fina pueden ayudar al diagno?stico ysu tratamiento definitivo es quirúrgico
