Management of blunt injuries of lower limbs with coexistent orthopaedic and vascular lesions is one of the greatest challenges in trauma surgery. These injuries are commonly seen in young men. The aim of this publication is to describe management of such injuries, based on the case report of a man with an extensive blunt trauma of the lower limb with coexistent injuries of the osteoarticular system and major blood vessels. Management of complex injuries of lower limbs requires team cooperation of vascular surgeons and traumatology specialists. Restoration of blood supply in the injured limb seems to be a priority but exceptions from this rule are possible in cases of recent limb ischaemia.
