Study of systemic fungal infections

This retrospective epidemiological study, was performed in theUniversity Hospital of Ioannina Northwestern Greece, during a 12 yearperiod (2001-2012).The purpose of this study was to determine the incidence ofcandidemia during the last 12 years, to investigate the causative agentswhich are responsible of its appearance and possible changes in thedistribution of Candida species isolated and their sensitivity to antifungal agents.During the study period, 211 different episodes of candidemia and 216blood yeast isolates were identified. Isolation of Candida spp from bloodcultures was performed by BacT/Alert automated system (BioMérieux,France). All isolates were identified using Vitek II system (BioMérieux)and API32C (BioMérieux). Antifungal susceptibility was determinedusing Vitek II system (BioMérieux) complemented with E-test(BioMérieux) and MICs were evaluated according to CLSI criteria.Detection of galactomannan antigen in the serum of 100 patients withpossible pulmonary aspergillosis, was performed using the PlateliaAspergillus enzyme immunoassay (Bio-Rad, Hercules, CA).According to our results there was no significant increase in frequencyof candidemia. During the first period of study (2001-2006) the rate was3,72%, while in the second period (2007-2012) the rate was 4,38%.In total, C.albicans was the most frequent species isolated (53,7%),followed by non-albicans species (46,3%).Regarding the adults, in the first six years of study, C.albicans was theprevailing species (59,5%), followed by non-albicans species (40,4%).In the same group of patients during the second period Candida nonalbicansspecies were the prevailing species representing 51,6%,followed by C.albicans species (48,3%).Respectively for newborns of all isolates, non-albicans speciesprevailed (59,5%), followed by C.albicans species (40,5%).Predominant risk factors for adults included the use of broad-spectrumantibiotics longer than 2 weeks, central venous and urinarycatheterization, total parenteral nutrition, colonization of Candida spp (≥2 positions), length of stay (LOS) in an intensive care unit ≥ 9 days, useof steroids, intra-abdominal surgery, cancer and chemotherapy,diabetes mellitus and hemodialysis. Predominant risk factors fornewborn included prematurity (gestational age 2μg/ml). All C.krusei and C.guilermondii aswell as 5 C.glabrata and one C.albicans isolates were found highlyresistant to fluconazole (MIC>64μg/ml). The majority of C.parapsilosisexhibited resistance to Echinocandins (MIC>4μg/ml).Detection of GM, as a diagnostic tool in aspergillosis, proved to bevery significant not only in patients with hematological malignancies butin patients with solid organ malignances as well. The sensitivity andspecificity of GM test was 100% and 83,3% respectively. In conclusion,detection of GM in combination with other diagnostic tests couldcontribute in accurate and early diagnosis of invasive aspergillosis.Finally, continuous surveillance is required in order to monitor trendsin incidence and changes in species distribution associated with fungalinfections in Greece.Αυτή η προοπτική μελέτη, πραγματοποιήθηκε σε χρονικό διάστημα 12ετών (2001-2012), στο Πανεπιστημιακό Γενικό Νοσοκομείο Ιωαννίνων.Σκοπός της ήταν η μελέτη καντινταιμιών, η συχνότητα εμφάνισής τουςκατά τη διάρκεια των 12 ετών, η διερεύνηση των αιτιολογικώνπαραγόντων, οι οποίοι είναι υπεύθυνοι γι’ αυτές, οι πιθανές αλλαγέςστην κατανομή των Candida spp που απομονώθηκαν και η ευαισθησίααυτών στους αντιμυκητιακούς παράγοντες.Η απομόνωση, η ταυτοποίηση και ο έλεγχος ευαισθησίας έγιναν με τααυτοματοποιημένα συστήματα BacT/Alert (ΒioMérieux), Vitek II(ΒioMérieux), API32C (ΒioMérieux), καθώς και με μεθόδους κλασικήςΜικροβιολογίας και με το E-Test (AB ΒioMérieux).Επίσης αξιολογήθηκε η δοκιμασία ανίχνευσης του αντιγόνου τηςγαλακτομαννάνης (GM), στον ορό αίματος 100 ασθενών με πιθανήπνευμονική ασπεργίλλωση. Ό έλεγχος έγινε με την Platelia Aspergillusenzyme immunoassay (Bio-Rad, Hercules,CA).Σύμφωνα με τα αποτελέσματά μας, κατά τη χρονική αυτή περίοδο,όσον αφορά τις καντινταιμίες, δεν παρατηρήθηκε σημαντική αύξηση στησυχνότητά τους. Την πρώτη εξαετία η συχνότητα αυτών ήταν 3,72%,ενώ τη δεύτερη ήταν 4,38% .Την πρώτη εξαετία επικράτησαν τα στελέχη C.albicans (59,5%) σεσχέση με τα non-albicans στελέχη (40,4%). Εν αντιθέσει, τη δεύτερηεξαετία επικράτησαν τα στελέχη non-albicans (51,6%) επί τωνC.albicans (48,3%). Για τα νεογνά, για όλο το χρονικό διάστημα των 12ετών, επικράτησαν τα στελέχη non-albicans (59,5%) σε σχέση με ταC.albicans (40,5%).Οι παράγοντες κινδύνου που προέκυψαν για τους ενήλικες, ήταν οιέξης: η λήψη ευρέος φάσματος αντιβιοτικών για χρονικό διάστημα ≥ 2εβδομάδων, η χρήση κεντρικού αγγειακού καθετήρα, η χρήσηουροκαθετήρα, η ολική παρεντερική σίτιση, ο αποικισμός από Candidaspp (≥ 2 θέσεων), η παραμονή σε Μ.Ε.Θ.( ≥ 9 ημέρες), η θεραπεία μεκορτικοστεροειδή, οι χειρουργικές επεμβάσεις, οι νεοπλασίες καιθεραπεία με κυτταροτοξικούς παράγοντες, ο σακχαρώδης διαβήτης καιη αιμοδιάλυση.
