Chemokines acting through CCR2 and CCR5 receptors in patients with type 2 diabetes mellitus and parameters of the metabolic syndrome

Background/Aims: The metabolic syndrome (MetS) comprises a cluster of abnormalities including insulin resistance, adiposity, dyslipidaemia and hypertension. It is associated with an increased risk for the development of type 2 diabetes (T2DM), cardiovascular disease and overall mortality. Low-grade chronic inflammation has been suggested to play a central role in the pathogenesis of the MetS, T2DM and atherosclerosis. Chemokines might exert a contributory role in this inflammatory process. Thus, the aim of the present study was to investigate whether four serum or plasma chemokines levels (MCP-1, MIP-1a, MIP-1b and RANTES) and two mutations of CCR2 and CCR5 receptors (CCR2-V64I and CCR5-A32) are associated with MetS and its complications. Materials and Methods: This study included 199 type 2 diabetic patients with MetS (group A, 96 male, mean age 65.6±9.2 years and mean diabetes duration 10.9 ± 8.5 years) and 109 healthy age-matched controls (group B, 51 men, mean age 64.4 ± 8.9 years). All subjects were genotyped for the CCR2-V64I polymorphism and CCR5- Δ32 mutation. Serum MCP-1, MIP-1a, MIP-1b and RANTES concentrations, as well as fasting glucose, HbA1c, serum lipids and uric acid were also measured. In addition, we measured waist and hip circumference, BMI and Ankle Brachial Index (ABI). Results: No association between the CCR5-A32 mutation and the MetS was found. However, the CCR2-V64I polymorphism was significantly (p=0.006) more frequent in patients with MetS and DM2. Serum MCP-1 and MIP-1b levels were significantly elevated in group A compared with group B (p<0.001 for both), while serum MIP-1a and plasma RANTES levels showed no differences between the two groups. In group A, MCP-1 concentration was positively correlated with HbA1c (rs=0.367, p<0.001) and negatively with ABI (rs=-0.278, p<0.001). Furthermore, patients whose ABI was diagnostic of peripheral arterial disease (PAD) showed significantly higher serum MCP-1, in comparison to those with MetS without PAD (p<0.001). Serum MIP-1a was positively correlated with waist circumference (rs=0.150, p=0.046), waist/hip ratio (rs=0.213, p=0.004) and uric acid concentration (rs=0.294, p=0.002). Serum MIP-1b was negatively correlated with HDL (rs=-0.188, p=0.045); in men, it was, additionally, positively correlated with uric acid levels (rs=0.291, p=0.042). Positive associations of MIP-1b levels with MIP-1a (rs=0.182, p=0.010) and (additionally, in women) with MCP-1 (rs=0.286, p=0.003) were also observed. Finally, plasma RANTES were negatively correlated with HDL (LogRANTES, r=-0.222, p=0.019) and fasting glucose levels (rs=-0.141, p=0.048). Conclusions: The present study demonstrated an association between the CCR2-V64I polymorphism and the MetS. It was also shown that T2DM patients with MetS exhibited significantly elevated serum MCP-1 and MIP-1b levels compared to age- matched healthy controls. Furthermore, all chemokines studied were significantly associated with parameters of MetS, markers of glycaemic control and indexes of cardiovascular complications. Taken together, these findings suggest that chemokines may contribute to the low-grade chronic inflammation, which is the common basis of MetS, obesity, T2DM and atherosclerosis. This effect is likely to be exerted by an action on the CCR2 and CCR5 receptors. Further prospective studies are required to delineate the precise role of chemokines in the generation and maintenance of this inflammatory process.Εισαγωγή/Σκοπός: Το Μεταβολικό Σύνδρομο (ΜΣ) αποτελεί μια ομάδα μεταβολικών διαταραχών, που περιλαμβάνουν την αντίσταση στην ινσουλίνη, την παχυσαρκία, την δυσλιπιδαιμία και την υπέρταση. Σχετίζεται με αυξημένο κίνδυνο για εμφάνιση Σακχαρώδη Διαβήτη τύπου 2 (ΣΔ2), καρδιαγγειακής νοσηρότητας και θνητότητας. Έχει προταθεί ότι μια χαμηλού βαθμού χρόνια φλεγμονώδης διαδικασία παίζει κεντρικό ρόλο στην παθογένεια του ΜΣ, του ΣΔ2 και της αθηροσκλήρωσης. Σε αυτή τη φλεγμονώδη διαδικασία, οι χημειοκίνες μπορεί να συνεισφέρουν σημαντικά. Υλικό και μέθοδοι: Στη μελέτη έλαβαν μέρος 199 διαβητικοί ασθενείς με ΜΣ (ομάδα Α, 96 άνδρες, μέσης ηλικίας 65.6 ± 9.2 ετών και μέσης διάρκειας διαβήτη 10.9 ± 8.5 έτη) και 109 υγιείς εθελοντές της ίδιας ηλικίας (ομάδα Β, 51 άνδρες, μέσης ηλικίας 64.4 ± 8.9 έτη). Σε όλους τους συμμετέχοντες έγινε γονοτυπικός έλεγχος για τον CCR2-V64I πολυμορφισμό και τη CCR5-Δ32 μετάλλαξη. Επίσης, μετρήθηκαν οι συγκεντρώσεις στον ορό των MCP-1, ΜΙΡ-1α και ΜΙΡ-1β, στο πλάσμα της RANTES, η γλυκόζη νηστείας, η HbA1c, το λιπιδαιμικό προφίλ, το ουρικό οξύ, η περίμετρος της μέσης, η περίμετρος στο ύψος των ισχύων, το ΒΜΙ και ο κνημοβραχιόνιος δείκτης (ΚΒΔ).
