El texto se encuentra organizado de la siguiente manera. En el primer capítulo se lleva a cabo la descripción anatómica y fisiológica de la deglución. De esta manera se comprenden los aspectos básicos y fundamentales que implican el acto de comer entendido desde la perspectiva médica. Así, se reconstruye el contexto clínico y el marco de referencia en términos de los conceptos y los problemas que se presentan en relación con la deglución. Principalmente en este punto se profundiza en los tipos de alteraciones que existen en la deglución y los tratamientos que existen para asistirlo y procurar la rehabilitación de dicha capacidad. Al finalizar este capítulo se matiza y delimitan los problemas, vacíos y dificultades que tienen los profesionales rehabilitadores de pacientes con disfagia para brindar atención y tratamiento adecuado y se identifican cuáles son los criterios mediante los cuales se valora su práctica. El segundo capítulo puede dividirse en dos apartados. En el primero, se lleva a cabo el análisis sobre el acto de comer, que engloba aspectos sociales, culturales y éticos. Este abordaje, que sigue la perspectiva bioética clínica, permitirá entender de qué manera los trastornos que afectan la deglución tienen impacto a nivel psicológico en los pacientes, y causan conflictos de tipo moral entre el médico y su paciente, así como en los cuidadores. Se considera clave el rol de los fonoaudiólogos para que, desde principios bioéticos, sustentados en una discusión interdisciplinar, brinden un trato humanizado a los pacientes, enfocados en lograr la recuperación integral de las capacidades fisiológicas de estos. En el segundo apartado de este capítulo, se realiza la exposición del enfoque de la bioética que permite comprender los problemas éticos que surgen en la relación médico paciente en la atención de la disfagia. Como base en dicha exposición, se presenta la reflexión del filósofo Michel Foucault y su explicación sobre lo que es la biopolítica y cómo esto ha dado luces para diversos análisis en bioética clínica. Al finalizar este capítulo, se concreta la noción de humanización como pilar de la atención en salud y como base sobre la cual pueden configurarse los criterios que deben orientar la práctica de los profesionales en fonoaudiología. Por ello, se propone hablar de la humanización en la práctica médica. En el tercer capítulo, se presenta el análisis de los resultados de la encuesta que se realizó a 49 profesionales en fonoaudiología, residentes en Colombia, en relación con las prácticas humanizadas que llevan a cabo en su lugar de trabajo, sea institución pública o privada. Para dicho análisis se utilizó el software Atlas.ti. En el último capítulo, que es el de las conclusiones generales de la investigación, se formularán una serie de criterios de valoración que orienten a los fonoaudiólogos a tomar decisiones y prácticas humanizadas en el tratamiento de la disfagia.Profesionales en FonoaudiologíaThe text is organized as follows. The first chapter contains an anatomical and physiological description of swallowing. In this way, the basic and fundamental aspects that involve the act of eating understood from the medical perspective are understood. At the end of this chapter, the problems, gaps and difficulties that rehabilitation professionals of patients with dysphagia have to provide adequate care and treatment are clarified and the criteria by which their practice is assessed are identified. The second chapter can be divided into two sections. In the first, the analysis of the act of eating, which encompasses social, cultural and ethical aspects, is carried out. This approach, which follows the clinical bioethical perspective, will allow us to understand how swallowing disorders have an impact on the psychological level in patients, and cause moral conflicts between the physician and the patient, as well as in the caregivers. The role of phonoaudiologists is considered key so that, based on bioethical principles, supported by an interdisciplinary discussion, they provide a humanized treatment to patients, focused on the integral recovery of the patients' physiological capacities. In the second section of this chapter, the bioethical approach that allows understanding the ethical problems that arise in the patient-physician relationship in dysphagia care is presented. As a basis for this exposition, the reflection of the philosopher Michel Foucault and his explanation of what biopolitics is and how this has shed light on various analyses in clinical bioethics is presented. At the end of this chapter, the notion of humanization is specified as a pillar of health care and as a basis on which the criteria that should guide the practice of professionals in speech therapy can be configured. Therefore, it is proposed to talk about humanization in medical practice. In the third chapter, we present the analysis of the results of the survey carried out to 49 professionals in phonoaudiology, residents in Colombia, in relation to the humanized practices that they carry out in their workplace, whether public or private institution. The Atlas.ti software was used for this analysis.
