Le processus de négociation dans la clinique psychanalytique : L'abîme du non-rapport médiatisé dans un atelier de musicothérapie

This thesis addresses a new research perspective considering the musical instrument as a privileged tool of the mediatization process for the transfer between the patients and the music therapist. This process happens in the course of music therapy workshops dedicated to psychotic patients hospitalized in psychiatry. Therefore, this approach differs from the classic therapeutic strategy, supported in particular by Édith Lecourt, in which the patient is in a passive position and the music produced in groups is supposed to cover the voices persecuting him. Conversely, we propose an improvisation device in which the patient is in an active position allowing him to connect with them. Starting from there, we support the idea that the relationship with the instrument requires a time of tuning because this relation is confronted with an impossible : the one imposed by the limits of the instrumentalist’s body but also the organological ones of the instrument. On this basis, we make the hypothesis that a musicotherapeutic device, whose fundamental rule is improvisation, creates the possibility to make this tuning time the scene of a negotiation between the psychotic and his superego vociferations expressed with more or less strong rhythms. Hence the challenge of this work is to define this negotiation process through the prism of psychoanalytic theory. In the context of our clinic, we define this process as the one by which the psychotic, because of the specific failure of the relationship to the instrument, tries to respond to the enigmatic, even superegoic call (“Enjoy/play!”), which comes from the place of the instrument. Therefore, from this perspective, the challenge of the music therapy device is to create the conditions allowing patients to experience a possible renegotiation of what did not happen during their access to the symbolism. And doing so without succumbing to the abyss of the superego accusations. This wasy the music therapy device will aim to attempt novel responses to the symptoms of patients, by creating a new type of relationship to the object (musical instrument).La thèse aborde une nouvelle perspective de recherche considérant l’instrument de musique comme un outil privilégié du processus de médiatisation du transfert entre les patients et le musicothérapeute. Ce processus intervient dans le cadre des ateliers de musicothérapie destinés à des patients psychotiques hospitalisés en psychiatrie. Cette approche se distingue donc de la stratégie thérapeutique classique, soutenue notamment par Édith Lecourt, dans laquelle le patient est dans une position passive et la musique produite en groupe a pour fonction de couvrir les voix qui le persécutent. Nous proposons, à l’inverse, un dispositif d’improvisation dans lequel le patient est dans une position active qui l’autorise à aller au contact de ces dernières. Sur cette base, nous posons que le rapport à l’instrument exige un temps d’accordage du fait que ce rapport soit confronté à un impossible : celui imposé par les limites du corps de l’instrumentiste mais aussi ceux organologiques de l’instrument. Partant de là, nous faisons l’hypothèse qu’un dispositif musicothérapeutique, dont la règle fondamentale est l’improvisation, balise la possibilité de faire de ce temps d’accordage la scène d’une négociation entre le psychotique et ses vociférations surmoïques. Ces dernières s’exprimant sous forme de rythmiques plus ou moins appuyées. L’enjeu de ce travail consiste à définir ce processus de négociation au prisme de la théorie psychanalytique. Dans le cadre de notre clinique, nous définissons ce dernier comme le processus par lequel le psychotique, du fait du ratage propre au rapport à l’instrument, tente de répondre à l’appel énigmatique, voire surmoïque (« jou(i)e ! »), qui lui vient du lieu de l’instrument. Selon cette perspective, l’enjeu du dispositif de musicothérapie est donc de créer les conditions permettant aux patients de faire l’expérience d’une possible renégociation de ce qui ne s’est pas passé dans l’accès au symbolique. Et cela, sans succomber dans l’abîme de l’accusation surmoïque. Ce faisant, le dispositif musicothérapeutique visera à tenter des réponses inédites face aux symptômes des patients, en créant un nouveau type de rapport à l’objet (instrument de musique)
