Funções executivas e atenção: análise em crianças com TDAH com e sem uso do Metilfenidato

Attention Deficit/Hyperactivity Disorder (ADHD) is a neuropsychological disorder that negatively affects patients' quality of life and affects about 3 to 7% of school-age children. The disorder has as symptomatological triad inattention, hyperactivity and impulsivity and is also related to deficits on executive functions (EF's), attention and verbal fluency. Methylphenidate is the most used psychostimulant in treatment of ADHD and there has been a growing expansion on consumption and prescription of this medication, a fact that may be related to the high consumption also by people who are not diagnosed, but seek cognitive improvement and to the greater amount of this disorder’s diagnoses. This increase in the use of methylphenidate and diagnosis of ADHD, as well as the controversies about the use, benefits and efficacy of the medication and the pressure that children undergo to have a good development on attention and EF's, are factors that lead to the necessity of more studies on ADHD, use of methylphenidate and its relationship with EF's, attention and verbal fluency. From this, the present study was designed in order to analyze the performance on executive functions and attention in children and/or adolescents with ADHD with and without use of methylphenidate. For this, this dissertation presents a first article of systematic review of researches that involve neuropsychological analysis of the performance profile on executive functions and attention of children and/or adolescents with ADHD using methylphenidate. In this, of the 71 publications found between 2010 and 2016, 6 studies were selected that met the inclusion criteria established in two distinct stages. It has been found that the efficacy of methylphenidate is better demonstrated on inhibitory control and cognitive flexibility, but there is a need for further studies regarding the effectiveness of the drug on working memory and attention. The second article of this dissertation is an empirical study that sought to analyze the three main executive functions abilities (working memory, inhibitory control and cognitive flexibility), the sustained, selective and alternated attention and the verbal phonological and semantic fluency of children with ADHD who use methylphenidate. The study included 71 children who were divided into three groups: 38 were part of the control group, 13 were part of the ADHD group with methylphenidate and 20 were part of the ADHD group without methylphenidate. After one-way ANOVA and correspondence analysis have been performed, it was concluded that children with ADHD using methylphenidate presented a lowered profile on executive functions, attention and verbal fluency. Finally, it is observed that there is no unanimity regarding a better or worse performance on executive functions, attention and verbal fluency of children with ADHD who use methylphenidate and stands out the need for new researches of this disorder and the use of methylphenidate, given the importance of knowing more about ADHD and its treatment so that the symptoms and risks caused by the disorder can be minimized and for clarification about the use and effect of the medication.NenhumaO Transtorno do Déficit de Atenção/Hiperatividade (TDAH) é um transtorno neuropsicológico que afeta negativamente a qualidade de vida dos pacientes e acomete cerca de 3 a 7% das crianças em idade escolar. O transtorno possui como tríade sintomatológica a desatenção, hiperatividade e impulsividade e também está relacionado a déficits nas funções executivas (FE’s), atenção e fluência verbal. O metilfenidato é o psicoestimulante mais utilizado no tratamento do TDAH e tem havido uma crescente expansão do consumo e prescrição deste medicamento, fato que pode estar relacionado ao grande consumo também por pessoas que não são diagnosticadas, mas buscam melhora cognitiva e à maior quantidade de diagnósticos do transtorno. Este aumento do consumo do metilfenidato e do diagnóstico de TDAH, bem como as controvérsias existentes acerca do uso, benefícios e eficácia do medicamento e a pressão que as crianças sofrem para que tenham bom desempenho atencional e em FE’s, são fatores que conduzem à necessidade de mais estudos sobre TDAH, uso do metilfenidato e sua relação com as FE’s, atenção e fluência verbal. A partir disso, o presente estudo foi delineado de modo a analisar o desempenho nas funções executivas e atenção em crianças e/ou adolescentes com TDAH com e sem uso do metilfenidato. Para isso, esta dissertação apresenta um primeiro artigo de revisão sistemática de pesquisas que envolvem análise neuropsicológica do perfil de desempenho em funções executivas e atenção de crianças e/ou adolescentes com TDAH em utilização do metilfenidato. Neste, das 71 publicações encontradas entre 2010 e 2016, foram selecionados 6 estudos que preencheram os critérios de inclusão estabelecidos em duas etapas distintas. Foi encontrado que a eficácia do metilfenidato é melhor comprovada no controle inibitório e na flexibilidade cognitiva, mas há a necessidade de mais estudos no que se refere a eficácia do medicamento na memória de trabalho e atenção. O segundo artigo desta dissertação é um estudo empírico que buscou analisar as três principais habilidades de funções executivas (memória de trabalho, controle inibitório e flexibilidade cognitiva), a atenção sustentada, seletiva e alternada e a fluência verbal fonológica e semântica de crianças com TDAH que usam o metilfenidato. Participaram do estudo 71 crianças que foram divididas em três grupos: 38 delas fizeram parte do grupo controle, 13 fizeram parte do grupo de TDAH com metilfenidato e 20 compuseram o grupo de TDAH sem metilfenidato. Após ser realizada ANOVA a um fator e análise de correspondência, concluiu-se que as crianças com TDAH em uso do metilfenidato apresentaram um perfil rebaixado de funções executivas, atenção e fluência verbal. Por fim, observa-se que não há unanimidade com relação a um melhor ou pior desempenho em funções executivas, atenção e fluência verbal de crianças com TDAH que usam metilfenidato e ressalta-se a necessidade de novas pesquisas sobre este transtorno e o uso do metilfenidato, dada a importância de conhecer mais sobre o TDAH e seu tratamento para que possam ser minimizados os sintomas e os riscos causados pelo transtorno e para que haja um esclarecimento acerca do uso e efeito do medicamento
