Les médecins généralistes confrontés à la maladie : prise en charge et vécu du statut de patient : enquête qualitative

Introduction: a doctor heals on a daily basis, but this caregiver status does not protect him from the disease. The interest shown to the doctor-patient increases with the years. This is due both to the evolution of medical demography and to the need to understand and improve the care of these particular patients. We wanted to hear these doctors speak out the disease, and gather their experience. How did they experience the onset of the disease, how was it managed? What was its personal and professional impact? And what are the peculiarities of this dual status of doctor and patient? Methods: we conducted semi-directed interviews with 12 general practitioners from Normandy suffering from from acute or chronic diseases. The qualitative analysis was obtained by thematic analysis of the verbatim. Results: the analysis revealed a virtual absence of an attending physician as such. The initial approach is overwhelmingly that of self-prescribing, both medicated and exploratory. Follow-up was frequently undertaken alone, the use of a specialist being a brake and needing to be justified. Patient status oscillated between a willingness to behave like a typical patient and the difficulty of giving up the role of a doctor and letting go. The position of health professional allowed some negotiation of care. One of the salient features of the study is the hierarchy that places work above health. It is explained by a professional culture of responsibility towards the patients, but also by the financial impact induced by a work stoppage. Conclusion: the sick doctor appears as a specific patient, both in his choice of care and the experience he has. To improve the health of doctors, solutions exist but must be promoted, while other proposals deserve to be studied.Introduction : le médecin soigne au quotidien mais ce statut de soignant ne le préserve pas de la maladie. L’intérêt porté au médecin-malade croit avec les années. Il s’explique tant par l’évolution de la démographie médicale que par le besoin de comprendre et d’améliorer la prise en charge de ces patients. Nous avons voulu entendre les médecins généralistes s’exprimer face à la maladie et recueillir leur expérience. Comment ont-ils vécu l’apparition de la maladie, comment a-t-elle été prise en charge ? Quel a été son impact, personnel et professionnel ? Et quelles sont les particularités du double statut de médecin et de patient ? Méthode : nous avons réalisé des entretiens semi-dirigés auprès de douze médecins généralistes normands ayant souffert ou souffrant de pathologies aiguës ou chroniques. L’étude qualitative a été obtenue par analyse thématique des verbatims. Résultats : l’analyse a révélé une quasi absence de médecin traitant utilisé comme tel. La démarche initiale était très majoritairement celle de l’autoprescription tant médicamenteuse qu’exploratoire. Le suivi était fréquemment entrepris seul, le recours à un spécialiste représentant un frein et devant être justifié. Le statut de patient oscillait entre une volonté de se comporter comme un patient lambda et la difficulté d’abandonner son rôle de médecin et de lâcher prise. La position de professionnel de santé permettait une certaine négociation des soins. On note parmi les résultats de l’étude un élément saillant : la hiérarchie plaçant le travail au-dessus de la santé. Elle s’explique par une culture professionnelle de responsabilité vis-à-vis des patients, mais également par l’impact financier qu’induit un arrêt de travail. Conclusion : le médecin malade apparaît comme un patient spécifique, tant dans ses choix de prise en charge que par le vécu qu’il expérimente. Pour améliorer la santé des médecins, des solutions existent mais doivent être promues, tandis que d’autres propositions méritent d’être étudiées
