Hiatal hernia clinical presentation and treatment

Reprezintă protruzia sau întoarcerea porțiunii superioare a stomacului în cavitatea toracică, prin orificiul diafragmatic care realizează comunicarea acesteia cu cavitatea abdominală. Scopul lucrării. Descrierea tipurilor, simptomelor, diagnosticului și metodelor de tratament chirurgical. Material și metode. Studierea articolelor științifice la tema dată. Rezultate. Simptomele herniei hiatale includ: senzația de arsură sau durere retrosternală, gust acru sau amar, disfagie, grețuri și vărsături. Exista patru tipuri cunoscute ale herniei hiatale: de alunecare (tip I), paraesofagiană (tip II), mixtă (tip III), cu elemente din primele și complexă (tip IV), când pe lângă stomac herniază și alte organe (colon, mare epiploon). Tipurile II, III si IV, sunt rar întâlnite și reprezintă 5%-15% din totalul herniilor hiatale. Pentru a preveni complicațiile e necesar un diagnostic adecvat. Diagnosticul este stabilit in baza examenului baritat al tractului gastrointestinal superior și endoscopia digestivă superioară. Tratamentul chirurgical consta în fundaplocatie prin intervenția laparoscopică. Concluzii. Hernia de tip I este cea mai frecventa. Tratamentul depinde de tipul, volumul, simptomele si complicațiile bolii.Background. Hiatal hernia is one of the most common diseases of the digestive tract. It represents the protrusion or return of the upper part of the stomach into the thoracic cavity, through the diaphragmatic orifice that makes its communication with the abdominal cavity. Objective of the study. Description of types, symptoms, diagnosis, and methods of surgical treatment. Materials and methods. Study of scientific articles on the given topic. Results. Symptoms of hiatal hernia include burning sensation or retrosternal pain, sour or bitter taste, dysphagia, nausea, and vomiting. There are four known types of hiatal hernia: sliding (type I), paraesophageal (type II), and mixed (type III), with elements of the first and complex (type IV), when in addition to the stomach herniates and other organs (colon, large epiploon). Types II, III and IV are rare and represent 5% -15% of total hiatal hernias. Adequate diagnosis is required to prevent complications. The diagnosis is established based on the barite examination of the upper gastrointestinal tract and upper digestive endoscopy. Surgical treatment consists of foundation by laparoscopic intervention. Conclusions. Type I hernia is the most common. Treatment depends on the type, volume, symptoms, and complications of the disease
