Manejo perioperatorio de la esclerosis múltiple

Multiple sclerosis (MS) is a demyelinating disease of the central nervous system (CNS), which affects young adults with a chronic clinical course. It is characterized by the appearance of neurological symptoms in the form of periodic outbreaks, which can be maintained and/or progress over time, causing residual disability.The perioperative management of patients with neurological diseases is usually complex, since it requires taking into account the alterations that anesthesia can cause on pre-existing dysfunctions, as well as the effect that the disease and its treatment have on the functioning of our drugs.As we will see later, the most important thing when caring for a patient with MS will be to have a good knowledge of the disease as well as to carry out a thorough preoperative evaluation. In this way they can be intervened safely.Here we present a brief review of the currently available evidence on the pathophysiology, clinical presentation, diagnosis, treatment, and anesthetic implications of patients with MS. To achieve this, a systematic bibliographic search of articles in the PubMed database has been carried out using the terms "multiple sclerosis", "anesthesia" and "perioperative".In this way, it is concluded that, with a thorough preoperative evaluation and a thorough knowledge of the disease, patients with MS can be operated on safely.La esclerosis múltiple (EM) es una enfermedad desmielinizante del sistema nervioso central (SNC), que afecta a adultos jóvenes con un curso clínico crónico. Se caracteriza por la aparición de síntomas neurológicos en forma de brotes periódicos, los cuales pueden mantenerse y/o progresar en el tiempo, provocando discapacidad residual. El manejo perioperatorio de los pacientes con enfermedades neurológicas suele ser complejo, ya que requiere tener en cuenta las alteraciones que la anestesia puede provocar sobre las disfunciones preexistentes, así como el efecto que la enfermedad y su tratamiento tienen sobre el funcionamiento de nuestros fármacos. Como veremos más adelante, lo más importante a la hora de atender a un paciente con EM será tener un buen conocimiento de la enfermedad así como realizar una exhaustiva evaluación preoperatoria. De esta forma podrán ser intervenidos de forma segura. Aquí presentamos una breve revisión de la evidencia disponible actualmente sobre las características fisiopatológicas, la presentación clínica, el diagnóstico, el tratamiento y las implicaciones anestésicas de los pacientes con EM. Para lograr esto, se ha realizado una búsqueda bibliográfica sistemática de artículos en la base de datos PubMed utilizando los términos “esclerosis múltiple”, “anestesia” y “perioperatorio”. De esta forma se llega a la conclusión de que, con una evaluación preoperatoria exhaustiva y un conocimiento profundo de la enfermedad, los pacientes con EM pueden ser intervenidos de forma segura
