Non-arteritic anterior ischemic optic neuropathy (NAION) may rarely develop following severe anemia and hypotension, usually secondary to acute blood loss, usually with a Hemoglobin of less than 7.5 g/dl. Here we report a case of NAION following a chronic history of moderate anemia and hypotension due to undiagnosed inflammatory bowel disease (IBD)
