Multiple myeloma is a hematologic malignancy characterized by clonal proliferation of plasma cells. While the disease is typically confined to the bone marrow, extramedullary involvement has been reported in approximately 5% of cases. Of these, orbital disease makes up a small subset. Here, we present a case of orbital plasmacytoma as the initial manifestation of multiple myeloma in a patient with a history of monoclonal gammopathy of unclear significance (MGUS)
