Use of botulinum toxin in the management of primary bruxism in adults: a systematic review.

A toxina botulínica (TB) tem sido usada para fins estéticos e terapêuticos há mais de 30 anos e tem mostrado bons resultados em pacientes com bruxismo, cujo controle com outras formas de tratamento tem sido mal sucedido. O objetivo desta revisão sistemática foi analisar os resultados clínicos obtidos com o uso de injeções de toxina botulínica tipo A (TB-A) no controle dos sintomas do bruxismo primário em adultos. Os bancos de dados PubMed, Web of Science, Scopus, LILIACS, Cochrane Library e Open Grey Literature foram pesquisados sem restrições de idioma e data até 6 de junho de 2021. Os estudos foram triados com o auxílio do software Mendeley Desktop. Um total de 741 referências foram obtidas nas 6 bases de dados nesta atualização da revisão sistemática sob o número CRD42019135511, PROSPERO 2019, sem financiamento. Os onze estudos clínicos randomizados controlados selecionados obtiveram redução dos sintomas do bruxismo, aplicando TB em 201 participantes com diferentes protocolos. O risco de viés foi avaliado com RoB 2.0 e o nível de evidência foi medido por meio da ferramenta GRADE. A heterogeneidade dos trabalhos incluídos não permitiu uma meta-análise. As injeções de TB-A são eficazes no tratamento dos sintomas de bruxismo primário em adultos. Mais ensaios clínicos randomizados são necessários para estabelecer um protocolo de uso da TB como alternativa às terapias tradicionais no manejo do bruxismo primário.Botulinum toxin (BT) has been used for aesthetic and therapeutic purposes for over 30 years and has shown good results in patients with bruxism, whose control with other forms of treatment has been unsuccessful. Studies were screened with the help of Mendeley Desktop software. A total of 741 references were obtained from the 6 databases in this systematic review update under number CRD42019135511, PROSPERO 2019, without funding. The eleven randomized controlled clinical trials selected achieved a reduction in bruxism symptoms, applying BT to 201 participants with different protocols. The risk of bias was assessed with RoB 2.0 and the level of evidence was measured using the GRADE tool. The heterogeneity of the studies included did not allow a meta-analysis. BTX-A injections are effective in treating symptoms of primary bruxism in adults. More randomized clinical trials are needed to establish a protocol for using BT as an alternative to traditional therapies in the management of primary bruxism.29f
