L'édentement subtotal : prise en charge et décision d'avulsion

Edentulism is a phenomenon that has always existed and which dental surgeons have always been confronted to. Prosthetic solutions have been widely described in the literature whether for partial or total edentulousness. Total edentulism, in the removable approach, is supported by total prostheses resting only on mucous support. However, subtotal edentulism is a situation that requires a special approach. Indeed, when the few remaining teeth can still be preserved, particularly in the mandibular arch, they can constitute an important additional support, stabilization and retention for a removable prosthesis. This approach, which is psychologically more acceptable for some patients, therefore makes it possible to obtain greater prosthetic comfort as well as to fit patients who would refuse to have their complete tooth loss “provoked”. It is therefore up to the dentist, by integrating parameters related to the patient, the arch concerned, the teeth concerned and his skills to propose this alternative when he considers that it has an interest for his patient.L’édentement est un phénomène qui a toujours existé et auquel les chirurgiens-dentistes ont toujours été confrontés. Les solutions prothétiques ont été largement décrites dans la littérature que ce soit pour les édentements partiels ou totaux. L’édentement total, dans l’approche amovible est pris en charge par des prothèses totales reposant uniquement sur un support muqueux. Cependant l’édentement subtotal est une situation qui requiert une approche particulière. En effet, lorsque les quelques dents restantes sont encore conservables notamment sur l’arcade mandibulaire, elles peuvent constituer complément important de sustentation, de stabilisation et de rétention pour une prothèse amovible. Cette approche qui est psychologiquement plus acceptable pour certains patients permet donc d’obtenir un plus grand confort prothétique ainsi que d’appareiller des patients qui refuseraient qu’on « provoque » leur édentement complet. Il revient donc au chirurgien-dentiste, en intégrant des paramètres liés au patient, à l’arcade concernée, aux dents concernées et à ses compétences de proposer cette alternative lorsqu’il considère qu’elle a un intérêt pour son patient
