This paper discusses the smallpox epidemic of 1797-1798 in the Villa of Orizaba and the towns of Maltrata, Aquila, and Ixhuatlancillo. It shows the aspects that contributed to the arrival of this disease. It highlights the Spanish Crown sanitary policy carried out by the subdelegation, the ayuntamiento, and the Church against this epidemic. On the social side, the study analyses how the State provided economic assistance to the affected population. Furthermore, it describes the demographic impact caused by the disease and compares it with the epidemic of 1779-1780.
