Actualización del manejo integral del hipotiroidismo subclínico

Hypothyroidism is a pathology characterized by high levels of the stimulating hormone called thyrotropin (TSH), on several occasions it manifests itself with mild symptoms, it precedes the subclinical term since, unlike the underlying disease, the hormonal levels of tetraiodothyronine and triiodothyronine are They are normal. Currently the decision of the correct management of the disease is a subject of debate, since an inadequate management of it can lead the patient to develop primary hypothyroidism, the frequency of thyroid diseases at present are causes of morbidity, mortality and disability at the worldwide, taking into consideration that hypothyroidism is the twenty-third most common diagnosis evaluated in medical consultation. Objective: To analyze the updated available literature on subclinical hypothyroidism, clinical manifestations, identification and comprehensive management. Method: Systematized Bibliographic Review of literature based on the selection of scientific articles from recent years based on the PRISMA method. Results: There is evidence of a clear clinical improvement in patients with T4 hormonal treatment, as long as the patient presents clear symptoms of the pathology, the benefit during pregnancy and in patients with comorbidities is significantly favourable, otherwise the analysis of the risks of the therapy is still in controversy. Conclusions: The treatment of subclinical hypothyroidism is totally individualized, pharmacological intake by hormone replacement with levothyroxine has shown significant benefits in symptomatic patients, the risk-benefit ratio will depend on the causes of said pathology according to the characteristics of the patient such as; race, sex and age.El hipotiroidismo es una patología caracterizada por niveles elevados de la hormona estimulante denominada tirotropina (TSH), en varias ocasiones se manifiesta con sintomatología leve, precede del término subclínico ya que a gran diferencia de la enfermedad de base los niveles hormonales de tetrayodotironina y triyodotironina se encuentran normales. Actualmente la decisión del correcto manejo de la enfermedad es un tema a debate, ya que un manejo inadecuado de la misma puede llevar al paciente a desarrollar un hipotiroidismo primario, la frecuencia de las enfermedades tiroideas en la actualidad son causas de morbimortalidad y discapacidad a nivel mundial, tomando en consideración que el hipotiroidismo es el vigésimo tercer diagnóstico más común evaluado en consulta médica. Objetivo: Analizar la literatura disponible actualizada acerca del hipotiroidismo subclínico, manifestaciones clínicas, identificación y manejo integral. Método: Revisión Bibliográfica Sistematizada de literatura basada en la selección de artículos científicos de los últimos años en base al método PRISMA. Resultado: Se evidencia una clara mejoría clínica en pacientes con tratamiento hormonal T4, siempre y cuando el paciente presente sintomatología clara de la patología, el beneficio durante el embarazo y en pacientes con comorbilidades es significativamente favorable, caso contrario el análisis de los riesgos de la terapéutica sigue en controversia. Conclusiones: El tratamiento del hipotiroidismo subclínico es totalmente individualizado, la ingesta farmacológica mediante reemplazo hormonal con levotiroxina ha demostrado beneficios significativos en pacientes sintomáticos, la relación riesgo – beneficio dependerá de las causas de dicha patología de acuerdo a las características del paciente como; raza, sexo y edad
