Síndrome de Weil, leptospirosis ictérica

Its diagnosis is serological (MAT) and / or culture of the bacteria, its differential is complex for the staff due to its varied clinical forms that occur with nonspecific symptoms, fever and / or jaundice without pathognomonic findings, having a wide range of pathologies with similar characteristics for which the epidemiological antecedent must be affiliated; and finally its treatment is based on intravenous penicillins in severe forms and orally in mild forms, and in allergic macrolides or tetracyclines depending on the age group. Objective: To describe an epidemiologically relevant clinical case in Ecuador such as Weil's syndrome, icteric leptospirosis. Material and Methods: Retrospective descriptive study, clinical case presentation Results: This is a young adult female patient with nopersonal history relevant to the case, debuts with flu symptoms associated with fever 39 ° daily without a predominance schedule, with an initial diagnosis of acute bronchitis receiving 5 days of amoxicillin plus clavulanic acid orally However, symptoms persist and are accompanied by jaundice, which is why complementary tests that reveal cholestatic pattern, hyperbilirubinemia are performed at the expense of direct and microscopic hematuria. Objetivo: Describir un caso clínico epidemiológicamenterelevante en Ecuador como el síndrome de Weil, leptospirosis ictérica. Material y Métodos: Estudio descriptivo retrospectivo, presentación de caso clínico Resultados: Se trata de una paciente femenina adulto joven sin antecedentes personales relevantes alcaso, debuta con cuadro de síntomas gripales asociados a fiebre 39° diaria sin horario de predominio, con diagnóstico inicial de bronquitis aguda recibe 5 días de amoxicilina más ácido clavulánico vía oral sin embargo síntomas persisten y se acompañan de ictericia por lo que se realizan complementarios que revelan patrón colestásico, hiperbilirrubinemia a expensas de la directa y hematuria microscópica, se estudian los diferenciales del síndrome febril ictérico inicialmente valorándose la colestasis extrahepática con un eco abdominal que reporta hígado de características normales, vesícula distendida sin cálculos, vía biliar sin alteraciones por lo que se investigan causas de colestasis intrahepática con virales para hepatotropos y no hepatotropos negativos, anticuerpos para hepatitis autoinmune negativos, aglutinaciones febriles negativas, hemocultivo negativo y por último al reinterrogar a la paciente nos revela el factor de riesgo laboral de contacto con roedores (auditora de bodega de almacenamiento) y viaje reciente a zona costera por lo que se realizan complementarios para investigar enfermedades tropicales con hematozoario negativo, IgM para dengue negativo y por último al 4to día de ingreso con diferenciales descartados, por antecedente epidemiológico, se estudia leptospira en sangre con ELISA y en orina con resultado positivo, tomando la muestra al día 7mo según la literatura y recibiendo el resultado al día 8vo. El cuadro sintomático se resolvió en 10 días y se envió con doxiciclina al alta al 12vo día. A los 10 días en control por consulta externa, paciente asintomática con perfil hepático normal. Conclusiones: Dada la presentación atípica del cuadro es subdiagnosticado, incrementándose así el número de desenlaces fatales en el 5-10% de la forma clínica icterohemorrágica, por lo que debe ser considerado en el diagnóstico diferencial de enfermedades tropicales y febriles ictéricas con el antecedente epidemiológico para iniciar el tratamiento oportuno
