Capsaicin: alternative topical agent in orofacial neuropathic pain

Orofacial neuropathic pain is extremely complex because its origins may be more complicated. The most common orofacial neuropathic pain disorders (such neuropathies, trigeminal neuralgia, postherpetica neuralgia, atypical odontalgia and complex regional pain syndromes), the symptoms, the possible etiology and the pharmacotherapy are reported. Particularly topical capsaicin has been used successfully to control of neuropathic pain. A systematic literature review on topic use in neuropathic pain of capsaicin was conducted. Capsaicin acts selectively on a subpopulation of local sensory neuron terminals depleting substance P and decreasing transmission of nociceptive impulses. The medication delivery and appropriate retention methods for oral and perioral diseases and pain control are described. Topical capsaicin is known to be a safe and effective pain management adjunct. Several studies have indicated the conditions that may benefit from capsaicin. Because of their rapid onset and low side effect profile, topical medications with capsaicin offer a vantage over conventional pharmacological therapies for orofacial neuropathic pain.
