Strategies in the elderly care in events of heat waves : A study of two municipality strategies in managing the negative effects of heat inelderly care

Även om utsläpp av växthusgaser skulle upphöra helt i nutid kommer tidigare mänskliga aktiviteter påverkaklimatet under många decennier. Väderextremer som värmeböljor har blivit mer frekventa och intensiva sedan1950-talet. För en befolkning som inte är anpassad till värme kan stigande temperaturer ge stora negativahälsoeffekter, inte minst för den äldre befolkningen. Syftet med studien är att undersöka vilka strategier två avVärmlands kommuner har för att hantera värmens negativa effekter inom äldreomsorgen vid värmeböljor samtatt undersöka den vårdnära personalens syn på värmeböljor som risk i äldreomsorgen. Vidare syftar studien tillatt utifrån James Resons barriärmodell The Swis cheese model undersöka om det finns några styrkor och svagheteri respektive kommuns strategi samt om de utgör ett tillräckligt starkt skydd för att hantera värmens negativaeffekter bland äldre. Metoden som användes var mixad metod, vilket innebar en kombination av både kvantitativtoch kvalitativt tillvägagångssätt. Det kvalitativa tillvägagångssättet var semistrukturerade intervjuer ochdokumentanalys av kommunens strategier och det kvantitativa var enkät som skickades ut till vårdnära personal.Både intervjuer och dokument analyserades utifrån en kvalitativ innehållsanalys. Enkäterna analyserades medhjälp av deskriptiv statistik. I studiens framkom att kommunerna har strategier för att hantera värmens negativaeffekter inom äldreomsorgen. I båda kommunernas respektive strategier framkom både styrkor och svagheter.Reasons Swiss Cheese Model var användbar när det gällde att identifiera styrkor och svagheter i strategierna. Defyra skyddsbarriärer som identifierade var: utbildning/kunskap, strategier, teknik och kommunikation. Det varframför allt i skyddsbarriären kommunikation som de allvarligaste svagheterna identifierades.Even if emissions of pollutions would cease in the present, past-human activities would still affect the climate formany decades. Weather extremes, such as heat waves have become more frequent and intense since the 1950s.For a population that is not adapted to heat, rising temperatures can have major negative health effects, especiallyfor the elderly population. The purpose of the study is to investigate what strategies two of Värmland'smunicipalities implement to manage the negative effects of heat in the elderly care during the event of heat wavesand to examine the healthcare staff views heat waves as a risk in elderly care. Furthermore, the study aims toinvestigate, based on James Reson's barrier model The Swis cheese model, whether there are any strengths andweaknesses in each municipality's strategy and whether they constitute a sufficiently strong protection to dealwith the negative effects of heat among the elderly. The method was a mixed method, which involved acombination of both quantitative and qualitative approaches. Two semi-structured interviews were conductedwith managers and a medically responsible nurse and a questionnaire was sent out to staff close to care. Theinterview material was analyzed on the basis of a qualitative content analysis. The questionnaires were analyzedusing descriptive statistics. A document analysis was also made of the municipalities' data for strategies based ona qualitative content analysis. The study indicate that the municipalities have strategies for dealing with thenegative effects of heat in elderly care. Both strengths and weaknesses emerged in both municipalities' respectivestrategies. Reason's Swiss Cheese Model was useful in identifying strengths and weaknesses in the strategies. Thefour protective barriers that were identified were: education / knowledge, strategies, technology andcommunication. The most serious weakness were identified in the protective barrier communication
