Intraoperative risk factors for the development of postoperative pain in endodontic treatment : a systematic review

A dor pós operatória em endodontia é desagradável tanto para o paciente como para o profissional. Diversos fatores já foram associados a esses eventos, tais como alterações do equilíbrio entre microorganismos e hospedeiro, erros na determinação do comprimento de trabalho, bem como a presença de microorganismos no sistema de canais radiculares. Neste contexto, o objetivo do presente estudo foi revisar, de forma sistemática, a literatura disponível acerca dos fatores de risco transoperatórios para o desenvolvimento de dor pós-operatória no tratamento endodôntico. Foram utilizadas as plataformas de busca PubMed, LILACS e Cochrane, com as seguintes palavras chaves: “endodontics” OU “root canal treatment” OU “root canal therapy” E “postoperative pain” OU “post-surgery pain” OU “post-treatment pain” OU “endodontic flare up” OU “endodontic pain” OU “postoperative endodontic pain”. Dois revisores independentes fizeram a análise dos títulos e resumos e, após, um revisor realizou a leitura e avaliou os resultados e a qualidade dos estudos incluídos na amostra. Ao total 21 artigos foram incluídos na revisão. Ao final da revisão concluímos que a utilização de instrumentos de níquel-titânio no preparo dos canais radiculares, no que diz respeito à menor ocorrência de dor pós operatória, está indicada. Além disso, a maioria dos estudos demonstra a equidade de incidência de dor em endodontias realizadas em sessão única ou múltipla. Entretanto, são necessários mais estudos para confirmar a influência de outros fatores transoperatórios do tratamento endodôntico na incidência de dor pós operatória.Postoperative pain in endodontics is unpleasant for both the patient and the professional. Several factors have already been associated with these events, such as changes in the balance between microorganisms and host, errors in determining the working length, as well as the presence of microorganisms in the root canal system. In this context, the objective of the present study was to systematically review the available literature on intraoperative risk factors for the development of postoperative pain in endodontic treatment. PubMed, LILACS and Cochrane search platforms were used, with the following keywords: “endodontics” OR “root canal treatment” OR “root canal therapy” AND “post-operative pain” OR “postsurgery pain” OR “post -treatment pain ”OR“ endodontic flare up ”OR“ endodontic pain ”OR“ postoperative endodontic pain ”. Two independent reviewers performed the analysis of the titles and abstracts and later a reviewer read and evaluated the results and the quality of the studies included in the sample. In total, 21 articles were included in the review. At the end of the review, we concluded that the use of nickel-titanium instruments in the preparation of root canals, with regard to the lower occurrence of postoperative pain, is indicated. In addition, most studies demonstrate similar incidence of postoperative pain in endodontics performed in a single or multiple session. However, further studies are needed to confirm the influence of other transoperative factors of endodontic treatment on the incidence of postoperative pain
