Deep brain stimulation for movement disorders

Diepe hersenstimulatie (DBS) is een behandeling waarbij met hoogfrequente elektrische stimulatie pathologische hersenactiviteit onderdrukt kan worden.Momenteel wordt DBS al zo’n 30 jaar toegepast, vooral bij patiënten met ernstige bewegingsstoornissen, zoals de ziekte van Parkinson, dystonie en tremor.Hoewel er duidelijk wetenschappelijk bewijs is voor de effectiviteit van DBS bij deze drie bewegingsstoornissen, is de effectgrootte van de behandeling nog beperkt, kan DBS de ziekten niet genezen en kan deze behandeling alleen toegepast worden bij een geselecteerde groep patiënten.In dit artikel gaan we in op de belangrijkste indicaties en contra-indicaties voor DBS en op de uitkomsten van de toepassing bij de genoemde bewegingsstoornissen.We bespreken de belangrijkste controverses en nieuwe ontwikkelingen op het gebeid van diepe hersenstimulatie om verwijzers en medebehandelaars een toegankelijke introductie tot de behandeling te bieden.3 gDeep brain stimulation (DBS) is a treatment which uses high-frequency electric stimulation to suppress pathological brain activity. DBS has been applied for over 30 years now, particularly in patients with severe movement disorders, such as Parkinson's disease, dystonia and tremor. Although there is clearly scientific evidence for the effectiveness of DBS in these three movement disorders, the effect size of the treatment remains limited. Furthermore, DBS is not curative and can only be applied in a select subset of patients. In this article, we discuss the key indications and contraindications for DBS, and the outcomes achieved when it is applied in the aforementioned movement disorders. We discuss the most notable controversies and new developments in the field of deep brain stimulation, in order to offer referrers and fellow healthcare professionals an accessible introduction to this mode of therapy.
