AÇÕES EDUCATIVAS DA EQUIPE DE SAÚDE À FAMÍLIA DA CRIANÇA COM CÂNCER E O CUIDADO PRESTADO NO ECOSSISTEMA DOMICILIAR: PERCEPÇÕES DA FAMÍLIA

The literature review was able to support the research, addressing the themes: Childhood cancer; The family of the child with cancer and the home ecosystem and educational health actions given to the family to care in the home ecosystem of the child with cancer. Methodology: This is a field research of the exploratory descriptive type, with a qualitative approach, which proved to be effective in all stages of the methodological path. The participants' search occurred in the hospitalization unit of children and adolescents with cancer at the Santa Maria University Hospital, located in the state of Rio Grande do Sul, Brazil. In the selection of the survey participants the inclusion criteria were adopted: to be family caregiver (parent or legal guardian) of children diagnosed and treated for cancer; Residing in municipalities in the state of Rio Grande do Sul. The following were excluded from the study: relatives of children not diagnosed with malignant neoplasm; children hospitalized with relapses; children in palliative care; and those residing in other states of the country. After the selection of the participants, the individual invitation to participate of the research was carried out, through the presentation of the Informed Consent Form, the research objectives, the verification of the possibility of conducting the research in the home environment and the authorization to record the interview in audio system. Only after acceptance to participate and signing the Consent and Informed, in two copies (one for the participant and one for the principal investigator) was scheduled, with the family, the date and time for the home interview, which was always performed after discharge of the child. After these steps, we set out to organize a home visit to families in the interior of Rio Grande do Sul to collect the data at the participants' residence, conducted in the months of March to August 2017. Data collection, in order to support the research question, assumptions, goals and key aspects of the literature review was conducted through semi-structured interviews and non-participant observation. For the analysis and interpretation of the data was used the method of Content Analysis in the modality of thematic analysis following the steps of Minayo. Results: The participants of this study were 14 family caregivers of children and adolescents with cancer. The results were discussed through two scientific papers. The first article entitled: Repercussions of childhood cancer in the family context evidenced the need to maintain research that focuses on this population, translating the results of these to practice, expanding knowledge and humanizing care by health professionals. The second article titled: Educational Actions to the Family of Children with Cancer for Home Care indicated the weaknesses in the guidelines on home care for families of children with cancer. It points out the need for health professionals play the role of health education during hospitalization of children with cancer and thus strengthen families for home care, impacting positively on the health of children with cancer. Product: The results of this research pointed out the need for informative material capable of assisting in home care, reducing the doubts that arise in the 9 post-hospital discharge. In this way, it was decided to develop a booklet, due to its easy handling, low cost and possible effectiveness. It includes important information to be observed in the care of children with cancer in the home, fosters understanding, and eliminates or lessens doubts and provides support in the realization of care. Thus, the final product of this dissertation consists of a booklet that contemplates the main guidelines capable of assisting family caregivers when providing care to the child and / or adolescent in cancer treatment in the home ecosystem.Objetivo: Elaborar o diagnóstico situacional, para aprofundar os conhecimentos acerca das ações educativas oferecidas à família, pela equipe multiprofissional durante o período de internação para o tratamento do câncer infantil, realizar o cuidado da criança no ecossistema domiciliar. A revisão de literatura realizada deu sustentação à pesquisa, abordando as temáticas: Câncer infantil; A família da criança com câncer e o ecossistema domiciliar e ações educativas em saúde oportunizadas à família para o cuidado da criança com câncer no ecossistema domiciliar. Metodologia: Trata-se de uma pesquisa de campo do tipo descritiva exploratória, com abordagem qualitativa, a qual mostrou-se efetiva em todas as etapas do caminho metodológico. A busca dos participantes ocorreu na unidade de internação de crianças e adolescentes com câncer do Hospital Universitário de Santa Maria, localizado no estado do Rio Grande do Sul/Brasil. Na seleção dos participantes da pesquisa observou-se como critérios de inclusão: ser cuidador familiar (pai ou mãe ou o representante legal), de criança diagnosticada e em tratamento contra o câncer; Residir em municípios do estado do Rio Grande do Sul. Foram excluídos do estudo: os familiares de crianças não diagnosticadas com neoplasia maligna; crianças internadas com recidivas; crianças em tratamento paliativo; e aquelas que residiam em outros estados do país. Após a seleção dos participantes, foi realizado o convite individual para participação da pesquisa, mediante apresentação do Termo de Consentimento Livre e Esclarecido, dos objetivos da pesquisa, da verificação da possibilidade de realizar a pesquisa no ambiente domiciliar e da autorização para gravação da entrevista em sistema de áudio. Somente após o aceite em participar da pesquisa e a assinatura do Termo de Consentimento Livre e Esclarecido, em duas vias (uma para o participante e a outra para o pesquisador principal) foi agendado, com o familiar, a data e o horário para a entrevista domiciliar, que foi realizada sempre após a alta da criança. Após essas etapas partiu-se para organização da visita domiciliar às famílias no interior do Rio Grande do Sul para a realização da coleta dos dados na residência dos participantes, realizadas nos meses março à agosto de 2017. A coleta de dados, no intuito de fundamentar a questão de pesquisa, pressupostos, objetivos e principais aspectos da revisão de literatura, foi realizada, por meio de entrevista semiestruturada e da observação não participante. Para a análise e interpretação dos dados foi utilizado o método da Análise de Conteúdo na modalidade da análise temática seguindo os passos de Minayo. Resultados: Os participantes desta pesquisa foram 14 cuidadores familiares de crianças e adolescentes com câncer. Os resultados foram discutidos por meio de dois artigos científicos. O primeiro artigo intitulado: Repercussões do câncer infantil no âmbito familiar evidenciou a necessidade de manutenção das pesquisas que focalizem essa população, traduzindo os resultados destes à prática, ampliando o conhecimento e humanizando o cuidado por parte dos profissionais de saúde. O segundo artigo intitulado: Ações educativas à família de criança com câncer para o cuidado domiciliar indicou as fragilidades nas orientações acerca do cuidado domiciliar às famílias de crianças com câncer. Aponta a necessidade dos profissionais de saúde desempenhar o papel de educadores em saúde durante a internação hospitalar de crianças com câncer e, assim, fortalecer as famílias para o cuidado domiciliar, repercutindo positivamente na saúde da 7 criança com câncer. Produto: Os resultados desta pesquisa apontaram a necessidade de material informativo capaz de auxiliar no cuidado domiciliar, reduzindo as dúvidas que surgem no pós-alta hospitalar. Desta maneira, optou-se por desenvolver uma cartilha, em razão de seu fácil manuseio, baixo custo e possível efetividade. Ela contempla informações importantes a serem observadas no cuidado à criança com câncer no domicilio, favorece a compreensão, elimina e ou ameniza dúvidas e fornece apoio na realização do cuidado. Assim, o produto final desta dissertação consiste em uma cartilha que contempla as principais orientações capazes de auxiliar os familiares cuidadores ao prestar o cuidado à criança e/ou adolescente em tratamento contra o câncer no ecossistema domiciliar
