Desempenho de métodos diretos e indiretos para o diagnóstico da Tuberculose bovina

É uma doença crônica, debilitante e normalmente assintomática por isso seu diagnóstico dependente de provas complementares. O Programa Nacional de Controle e Erradicação da Brucelose e Tuberculose (PNCEBT) prevê as tuberculinizações como únicos testes para o diagnóstico ante-mortem, além do isolamento e da identificação do agente como diagnóstico post-mortem. Segundo o PNCEBT, outros testes diagnósticos poderão ser adotados pelo programa desde que devidamente avaliados. O presente trabalho teve como objetivo a avalição do desempenho de três testes diagnósticos post-mortem aplicados em amostras com lesões sugestivas de tuberculose oriundas do Serviço de Inspeção Federal (SIF), além da avaliação do teste ELISA IDEXX e da tuberculinização cervical comparativa (TCC) em dois rebanhos sabidamente tuberculosos, que apresentavam diferentes prevalências e se encontravam em diferentes fases de controle da enfermidade, utilizando-se modelos Bayesianos de classe latente. Segundo os resultados obtidos, todos os testes avaliados apresentaram boa especificidade. O isolamento apresentou maior sensibilidade que os demais testes avaliados para diagnóstico post-mortem da tuberculose bovina. O teste ELISA IDEXX foi o menos sensível mas foi capaz de identificar animais reagentes que os demais testes não o fizeram, e a PCR em tempo real (qPCR), apresentou sensibilidade intermediária entre aquelas duas provas. Não foi encontrada correlação entre nenhum dos testes. A sensibilidade do teste ELISA IDEXX foi maior no rebanho de menor prevalência, que já se encontrava em fase de controle da enfermidade, em comparação com aquele onde não havia sido realizada nenhuma tuberculinização prévia e apresentava maior prevalência da enfermidade. O desempenho da TCC não variou entre os rebanhos avaliados. De acordo com os resultados obtidos, recomenda-se a utilização em série dos testes ante-mortem avaliados nesse estudo.Bovine tuberculosis is an infectious and contagious disease caused by M. bovis, with recognized negative impact on livestock production and risks to public health. It is a chronic, debilitating and usually asymptomatic disease so its diagnosis depends on complementary evidence. The National Program for the Control and Eradication of Brucellosis and Tuberculosis (PNCEBT) recommends the tuberculinizations as the only tests for the diseases ante-mortem diagnosis, besides the isolation and identification of the agent as a post-mortem diagnosis. According to the PNCEBT, other diagnostic tests may be adopted by the program provided that they are duly evaluated. The objective of the present study was to evaluate the performance of three post-mortem diagnostic tests applied in samples with suggestive lesions of tuberculosis from the Federal Inspection Service (SIF), as well as the ELISA IDEXX test and comparative cervical tuberculinization (CBT), as ante-mortem diagnostic tests, in two tuberculous herds, which presented different prevalences and were in different stages of disease control, using latent-class Bayesian models. According to the results obtained, all the tests evaluated presented good specificity. Isolation was more sensitive than the other tests evaluated for post-mortem diagnosis of bovine tuberculosis. The ELISA IDEXX test was the least sensitive but was able to identify reagent animals that the other tests did not, and real-time PCR (qPCR) showed intermediate sensitivity between the two tests. No correlation was found between any of the tests. The sensitivity of the IDEXX ELISA test was higher in the lower prevalence herd, which was already in the control phase of the disease, in comparison with that herd where no prior tuberculinization had been performed and showed a higher prevalence of the disease.
