Aspergerov sindrom - validnost dijagnostičkog koncepta

This paper presents actual controversy regarding diagnosis, differential diagnosis and status of Asperger syndrome today. Reasons for support distinct diagnostic categories as well as spectrum notion of autism are discussed. There are differences across diagnostic systems including whether Asperger syndrome is used as a term to denote milder difficulties than autism, whether neuropsychological profiles are assumed to be important, wheather the nature of the social deficit is considered to be different to that in autism. Several emerging lines of data are suggestive of some differences from autism in predictions of outcome, co-morbidity with other psychiatric disorders, neuropsychological profiles, family genetics or response to treatment. They all would be potential external validaters if they were not used as part of selection criteria in the first place.Ovaj rad pokazuje aktualne kontroverze u pogledu dijagnoze, diferencijalne dijagnoze i prirode Aspergerovog sindroma danas. Razmatraju se mogući razlozi za izdvajanje Aspergerovog sindroma kao posebnog kliničkog entiteta i razlozi za njegovo ostajanje u okviru spektra autističnog poremećaja. U sistemima dijagnostikovanja postoje razlike u pogledu odgovora na pitanje da li se termin Aspergerov sindrom koristi da označi blaže teškoće od onih kod autizma, da li se neuropsihološki profili smatraju značajnim i da li se pretpostavlja da se socijalni deficiti razlikuju kod Aspergerovog sindroma i autizma. Pojavljuje se nekoliko linija podatka koji sugerišu postojanje razlika između Aspergerovog sindroma i autizma, u pogledu predviđanja ishoda, komorbiditeta sa drugim psihijatrijskim poremećajima, neuropsiholoških profila, porodične genetike ili reagovanja na tretman. Ukoliko se ne koriste kao kriterijumi za selekciju ovi bi podaci mogli da posluže za empirijsku validaciju
