Efficacité des traitements basés sur l'imagerie motrice dans la prise en charge du Syndrome Douloureux Régional Complexe chez l'adulte

Introduction: Complex Regional Pain Syndrome is a pathology generally affecting a limb following trauma or injury. It manifests as an aberrant inflammatory response, the exact origin of which is still poorly understood. This affection causes multiple symptoms including chronic neuropathic pain that is largely debilitating for patients. Treatments such as Graded Motor Imaging (GMI) or mirror therapy are based on the hypothesis of a reorganization of the primary somatosensory cortex by CRPS. Therefore, motor imagery by brain training may normalize this change and thus decrease the pain and other symptoms of CRPS. Objectives: The objective of this review is to study the effectiveness of treatments based on motor imagery in the management of patients with CRPS. Method: 3 scientific databases were investigated: PubMed, Cochrane Library and PEDro. The research strategy developed “a priori” allowed us to study 6 randomized controlled trials. After evaluating their quality in terms of methodology, extracting the data and analyzing the results, we were able to write a synthesis. Results: 2 trials compared GMI with a control treatment and the other 4 with mirror therapy. The studies were heterogeneous, small sized, and all of them did not study the same outcomes with identical measurement tools. However, treatments based on motor imagery appear to have a beneficial effect on pain as well as motor function in patients with CRPS. Conclusion: The improvement in the condition of patients induced by these treatments constitutes an encouraging progress in its use. In addition, they do not bring constraints for patients as well as for therapists. However, there remains a lack of evidence due to the heterogeneity of the studies as well as their low number of participants.Introduction : le Syndrome Douloureux Régional Complexe est une pathologie affectant généralement un membre à la suite d’un traumatisme ou d’une lésion. Elle se manifeste par une réponse inflammatoire aberrante dont l’origine exacte reste encore mal connue. Cette affection provoque de multiples symptômes dont une douleur neuropathique chronique largement invalidante pour les patients. Les traitements comme l’imagerie graduelle motrice (IGM) ou la thérapie miroir se basent sur l’hypothèse d’une réorganisation du cortex somatosensoriel primaire par le SDRC. De ce fait l’imagerie motrice par l’entrainement cérébral pourrait normaliser cette modification et ainsi diminuer la douleur et d’autres symptômes du SDRC. Objectifs : L’objectif de cette revue est d’étudier l’efficacité des traitements basés sur l’imagerie motrice dans la prise en charge des patients atteints de SDRC. Méthode : 3 bases de données scientifiques ont été investiguées : PubMed, Cochrane Library et PEDro. La stratégie de recherche élaborée à priori nous a permis d’étudier 6 essais comparatifs randomisés. Après évaluation de leur qualité méthodologique, extraction des données et analyse des résultats, nous en avons rédigé la synthèse. Résultats : 2 essais comparaient l’IGM à un traitement contrôle et les 4 autres la thérapie miroir. Les études étaient hétérogènes, de petite taille et n’étudiaient pas toutes les mêmes critères de jugement avec des outils de mesure identiques. Toutefois les traitements basés sur l’imagerie motrice semblent avoir un effet bénéfique sur la douleur ainsi que la fonction motrice des patients atteints de SDRC. Conclusion : l’amélioration de la condition des patients induite par ces traitements constitue une piste encourageante dans son utilisation. De plus ils sont peu contraignants pour les patients comme pour les thérapeutes. Cependant il subsiste un manque de preuve dû à l’hétérogénéité des études ainsi qu’à leur faible nombre de participants
