The effect of dynamic lumbar stabilization exercises and respiratory muscle training on dialysis adequacy in hemodialysis patients

Çalışmanın amacı hemodiyaliz hastalarında dinamik lomber stabilizasyon egzersizleri (DLSE) ve solunum kas eğitiminin (SKE), fiziksel aktivite seviyesi, diyaliz yeterliliği ve yaşam kalitesi, solunum fonksiyon ve kas gücü üzerine etkisini incelemektir. Çalışmanın ikincil amacı, olgulardaki rezidüel böbrek fonksiyonu ve hemodiyalizin akut komplikasyonlarına etkisinin değerlendirilmesidir. 25-45 yaş aralığında, en az 6 aydır haftanın 3 günü 4'er saat hemodiyaliz tedavisi gören, 56 hasta randomize olarak 3 gruba ayrıldı. DLSE (n=17) ve SKE grubu (n=19) olguları 10 hafta boyunca haftanın 3 günü fizyoterapist gözetiminde intradiyalitik egzersiz programına katıldı. Kontrol grubunda (n=20) olağan nefrolojik terapiye devam edildi. Ölçümler egzersiz programına başlamadan önce ve 10. haftanın sonunda yapıldı. Katılımcıların yaşam kaliteleri Kidney Disease Quality of Life-36 yaşam kalitesi anketi, fonksiyonellik ve fiziksel aktivite seviyesi Altı Dakika Yürüme Testi, Zamanlı Kalk Yürü Testi, Beş Kere Otur Kalk Testi, Uluslararası Fiziksel Aktivite Anketi ve kişisel bilgi formundaki yaşam alışkanlıklarının sorgulanması ile elde edildi. Solunum sistemine olan etkilerin belirlenebilmesi için solunum fonksiyon testi (SFT), maksimal inspiratuar ağız içi basıncı (MIP) ve maksimal ekspiratuar ağız içi basıncı (MEP) ölçümleri yapıldı. Tedavi sonunda grupların diyaliz yeterliliğinde ve hemodiyalizin akut komplikasyonlarında anlamlı (p>0.05) bir değişim bulunmadı. DLSE grupta diğer gruplara göre günlük miksiyon sayısı ve idrar miktarı anlamlı olarak arttı. Egzersiz gruplarında kontrol grubundan farklı olarak sigara kullanım sıklığı artış eğilimi gösterdi, SFT, MIP, MEP, fiziksel aktivite seviye göstergelerinde, yaşam kalitesinin hastalık semptom ve problemler ile fiziksel komponentlerinde anlamlı bir artma oldu.The aim of this study was to investigate the effect of dynamic lumbar stabilization exercises (DLSE) and respiratory muscle training (RMT) on physical activity level, dialysis adequacy and quality of life, respiratory function and muscle strength. The second aim of the study was to evaluate the residual kidney function and the effect of hemodialysis on acute complications among the cases. Fifty six patients aged 25-45 years old who underwent hemodialysis treatment for 4hours a day for at least 6 months were divided randomly into 3 groups. The DLSE group (n=17) and RMT group (n=19) participated in the intradialytic exercise program under physiotherapist supervision for 3 days a week for 10 weeks. The usual nephrological therapy was continued in the control group (n=20). The measurements were done before the start of the exercise program and at the end of the 10th week. Participants' quality of life was obtained by the Kidney Disease Quality of Life-36 questionnaire, Functionality and Physical Activity level was obtained by Six Minute Walking, Timed Up and Go, and Five Times Sit-to-Stand test, International Physical Activity Questionnaire. In order to determine the effects on the respiratory system, Respiratory function test (RFT), maximal inspiratory mouth pressure (MIP) and maximal expiratory mouth pressure (MEP) measurements were performed. At the end of the treatment, there was no significant difference (p> 0.05) in dialysis adequacy of the groups and acute complications of hemodialysis. In the DLSE group, the number of daily urination and the amount of urine increased significantly compared to the other groups. Unlike the control group, the frequency of smoking tended to increase in the exercise groups and there was a significant increase in RFT, MIP, MEP, physical activity level indicators, disease symptoms, problems and physical components of the quality of life
