Zakrzepica w stencie u pacjenta z NSTEMI leczonego PCI z implantacją stentów uwalniajacych lek w trakcie infekcji COVID 19.

We present the case of a 67-year-old patient suffering from COVID 19, admitted to the Deparmet of Cardiology due to a heart attack - NSTEMI. A patient treated with PCI with stent implantation in GPZ and GD1 as antiplatelet therapy had the recommended acetylsaic acid and clopidogrel. In addition, he received enoxaparin throughout his hospitalization. In 2 days after discharge, the patient was re-admitted due to a heart attack - STEMI. In coronary annography, stent thrombosis in GPZ was found. The patient was modified antiplatelet therapy - replacing clopidogrel with ticagrelor, given eptifibatide and performed aspiration thrombectomy and balloon angioplasty. Due to the allergic reaction after ticagrelor, another conversion of antiplateplet therapy (switching from ticagrelor to prasugrel) was necessary.Przedstawiamy przypadek 67-letni pacjenta chorującego na COVID 19, przyjętego do OIOK z powodu zawału serca - NSTEMI. Pacjent leczony PCI z implantacją stentów w GPZ i GD1 jako leczenie przeciwpłytkowe miał zalecany kwas acetylosaoicylowy i klopidogrel. Ponadto, przez cały okres hospitalizacji otrzymywał enoksaparynę. W 2 dni po wypisie pacjent został ponownie przyjęty z powodu zawału - STEMI. W koronarografii stwierdzono zakrzepicę w stencie w GPZ. Pacejntowi zmodyfikowano leczenie przeciwpłytkowe - zamiana klopidogrel na tikagrelor, podano eptifibatyd i wykonano trombectomię aspiracyjną i angiopalstykę balonową. Ze względu na reakcję alergiczną po tikagrelorze konieczna była kolejna konwersja leczenia przeciwpłytkowego (zamiana tikagrelor na prasugrel)
