Functional imaging of the brain alteration in patients with chronic pain

Catedra Neurologie USMF “Nicolae Testemiţanu” Institutul de Neurologie şi NeurochirurgieRecently, local morphologic alterations of the brain in areas ascribable to the transmission of pain have been detected in patients suffering from phantom pain, chronic back pain, fibromyalgia and two types of frequent headaches. These alterations were different for each pain syndrome, but overlapped in the cingulate cortex, the orbitofrontal cortex, the insula and dorsal pons. These regions function as multi-integrative structures during the experience and the anticipation of pain. It seems that chronic pain patients have a common ‘‘brain signature” in areas known to be involved in pain regulation, the question arises whether these changes are the cause or the consequence of chronic pain. Some of the authors suggest that the gray matter change observed in chronic pain patients is the consequence of frequent nociceptive input and should thus be reversible when pain is adequately treated. Migraine is a type of chronic pain, and is taken for considering pathogenesis of chronic pain. Recent, leziunile locale morfologice ale creierului în ariile descrise ca fiind responsabile de transmiterea durerii, au fost observate la pacienţii ce suferă de:durerea fantomă, rahialgiile, fibromialgia, durerea de cap de tip tensional şi migrena. Aceste leziuni erau diferite pentru fiecare tip de durere în parte, dar se suprapun în cortexul cingulat, cortexul orbito-frontal, insula şi puntea dorso-laterala. Se pare că aceste regiuni sunt structuri multi –integrative în timpul experienţei şi anticipării durerii. Deci pacienţii cu durere cronică au o,,semnatură a creierului” comună, în ariile implicate în controlul durerii, întrebarea ce apare este:aceste leziuni sunt cauze sau consecinţe ale durerii cronice. Unii autori sugereaza faptul că schimbările corticale observate la pacienţii cu durere cronică sunt consecinţe a dereglării căilor nociceptive, de aceea ar trebui să fie reversibile, atunci cînd durerea este tratată adecvat. Migrena este o forma a durerii cronice, care este luată în consideraţie pentru a explica morfopatogeneza durerii cronice
