Hemostaza endoscopică prin metoda mecanică în managementul leziunii Dieulafoy

Background. According to the latest statistics, Dieulafoy’s lesion(DL) is responsible for up to 5% of acute gastro-intestinal (GI) bleeds. Unfortunatelly, there is no consensus on the optimal therapeutic option now. Even endoscopic management is considered effective tactics, the best method is still unclear. Objective of the study. To offer an overview of current data on available endoscopic techniques used for patients with GI bleeding resulting from DL. Material and Methods. The articles published during the years 2015-2020 from the PubMed database were selected according to the following keywords: “Dieulafoy’s lesion”, “Endoscopic hemostasis”, “Mechanical hemostatic techniques”, “Endoscopic band ligation”, “Endoscopic hemoclipping”. Results. Each endoscopic method has both advantages and disadvantages, however, mechanical therapies including endoscopic hemostatic clipping and band ligation (EBL) are considered to be the first option in the control bleeding from DL with a success rate of about 90%. In addition, studies show that patients treated with clipping and EBL have lower potential of recurrent bleeding with a lower mortality rate and excellent long-term results. Conclusion. Mechanical hemostatic therapy demonstrated good clinical outcomes compared with other endoscopic techniques and is recommended as effective option in patients with DL. Introducere. Cercetările recente confirmă că leziunea Dieulafoy(LD) provoacă până la 5% din hemoragiile gastrointestinale(GI). Actualmente nu există un consens asupra opțiunii terapeutice optime. În timp ce tratamentul endoscopic este considerat o tactica eficientă, cea mai bună metodă rămâne discutabilă. Scopul lucrării. Scopul lucrării reprezintă revizuirea datelor existente privind metodele endoscopice disponibile utilizate pentru pacienții cu hemoragie gastrointestinală cauzată de DL. Material și Metode. Din baza de date PubMed au fost selectate articole publicate recent (aa. 2015- 2020) după următoarele cuvinte-cheie: „Leziunea Dieulafoy”, „Hemostază endoscopică”, „Metode mecanice de hemostază”, „Ligaturarea endoscopică cu benzi”, „Endoclipse pentru hemostază”. Rezultate. Fiecare metodă endoscopică are atât avantaje, cât și dezavantaje, cu toate acestea, hemostaza mecanică, inclusiv bandarea endoscopică (BE) și clamparea endoscopică (CE), sunt considerate drept prima opțiune pentru hemoragie din LD cu o rată de succes de aproximativ 90%. În plus, studiile confirmă că pacienții cu BE și CE au potențiale mai reduse de recurență a hemoragiei cu rate de mortalitate mai mici și rezultate excelente la distanță. Concluzii. Terapia mecanică a demonstrat rezultate clinice bune comparativ cu alte tehnici endoscopice și este recomandată ca opțiune eficientă pentru pacienții cu LD
