HUBUNGAN RIWAYAT PEMBERIAN SEFALOSPORIN DENGAN KEJADIAN KOMPLIKASI APPENDISITIS AKUT PADA ANAK AKIBAT KETERLAMBATAN APPENDEKTOMI

Background : Appendicitis is the most common cause of acute abdominal pain that require surgical intervention, and the most common reason for abdominal surgery. Appendectomy as a standard management for non-perforated appendicitis is not complications free. There are many surgical risk such as intestinal obstruction which happened in 1.3 % in 30 years after appendectomy. Negative appendectomy happened in 10 % - 20 % of cases. In the present time, the non operative management of acute appendicitis without complications which treated with antibiotics is not well known yet, although there is evidence that this would break the habit and some research has shown for having the same results compared to appendectomy. Objective : Analyzing the relationship between history of cephalosporin administration with the acute appendicitis complications in children. Methods : This is an analytic observational study with case control approach to know correlation of cephalosporin administration history with the acute appendicitis complications in children. The data were analyzed descriptively using estimated relative risk then processed using 17.0 SPSS with chi-square (α=0,05) and served in 2x2 table. Results : The sample cosist of 42 patients who met the inclusion and exclusion criteria. There are 21 acute appendicitis patients with complications and 21 patients without complications. In a group of patients with complications, 13 patients were treated with cephalosporin and 8 patients were not treated with cephalosporin. In the group of patients with acute appendicitis without complications, 7 patients were treated with cephalosporin and 14 patients were not treated with cephalosporin. Risk of complications in patients which treated with antibiotics before referred to the hospital are being found (OR = 3.25). However, the increased risk was statistically insignificant (p = 0,064) Conclusions : History of antibiotics administration in acute appendicitis patients before referred to the hospital will increase the risk of complications in children (OR = 3.25), but are not statistically significant (p>0.05).
