Experiences of people with kidney transplantation.

The chronic kidney insufficiency consists in a loss of functions performed by kidneys, carrying to serious problems in person’s health. Among the adopted treatments, the transplantation sets up itself in the best therapeutic modality once it provides quality of life. Even when successfully carried on, it doesn’t mean cure, the person still being carrier of kidney disease. In this way, the person becomes dependent of care which aims the health maintenance. This study aimed to know the experiences of people with kidney transplantation through the Technique of Critical Incidents, having a qualitative approach, descriptive. Twenty people with kidney transplantation were interviewed, during the period of May to July of 2013. The approach was performed through telephone contact, in order to invite the transplanted people to participate of the study. After accepted, the meet were scheduled. Data collection occurred through semi structured interviews. After this, it was performed the content analysis, through a thorough and continuous reading, aiming to isolate the occurred situation, emitted behaviors and the results consequences, classifying them as a positive form and/or negative one. There were 683 critical incidents. Of these, 450 situations (272 positives and 178 negatives), 141 behaviors (108 positives and 33 negatives) and 92 consequences (19 positives and 73 negatives). In front of these findings, it was observed that the transplantation promotes an improvement in the quality of life, once it releases the person with chronic kidney insufficiency from dialysis. Moreover, it allows the return to the social, labor and intellectual activities, which were interrupted after the “coming” of the disease, improving in this way the physical and psycho posture. However, the person is not off of the kidney disease, once she needs to take new cares with her health, paying attention to food and fluid intake, to hygiene, to immunosuppressive drugs, further the restrictions in some activities and about the constant clinical reviews, living with the risk of organ transplanted rejection and the possibility of return to de dialysis. As worrying factors that can cause limitations in life, as well losses in the maintenance of health, it was highlighted the presence of collateral effects of immunosuppressive drugs, lack of guidelines about how behave in certain occasions and the deficient assistance of the health system. Upon the occurrence of factors, positives as negatives, implicated in the life of a person with kidney transplantation, it is believed that the Technique of Critical Incidents used in this study contributed to the knowledge of the person’s living.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESA insuficiência renal crônica consiste na perda das funções realizadas pelos rins, levando a graves problemas na saúde da pessoa. Entre os tratamentos adotados, o transplante configura-se na melhor modalidade terapêutica por proporcionar qualidade de vida. Mesmo o sendo bem sucedido, ele não significa a cura, a pessoa continua a ser portadora da doença renal. Assim, ela passa a depender de cuidados que visam à manutenção da saúde. Este estudo objetivou conhecer as vivências das pessoas com o transplante renal por meio da Técnica dos Incidentes Críticos, possuindo caráter qualitativo, do tipo descritivo. Foram entrevistadas 20 pessoas com o transplante renal, durante o período de maio a julho de 2013. A abordagem foi realizada por meio do contato telefônico, no intuito de convidar os transplantados a participar do estudo. Após o aceite, foram agendados os encontros. A coleta de dados ocorreu por meio de entrevista semiestruturada. Após, realizou-se a análise de conteúdo, por meio de uma leitura minuciosa e contínua, de modo a isolar a situação ocorrida, os comportamentos emitidos e as consequências resultantes, classificando-os de forma positiva e/ou negativa. Houve 683 incidentes críticos. Desses, 450 situações (272 positivas e 178 negativas), 141 comportamentos (108 positivos e 33 negativos) e 92 consequências (19 positivas e 73 negativas). Perante esses achados, observou-se que o transplante promove uma melhora na qualidade de vida, ao libertar a pessoa com insuficiência renal crônica do tratamento dialítico. Ademais, permite o retorno às atividades sociais, laborais e intelectuais, que foram rompidas após o estabelecimento da doença, melhorando assim, o estado físico e psicológico. Entretanto, a pessoa não está livre da doença renal, uma vez que passa a adotar novos cuidados com a saúde, atentando à alimentação, à ingesta hídrica, à higiene, aos medicamentos imunossupressores, além das restrições em algumas atividades e das constantes revisões clínicas, convivendo ainda, com o risco da rejeição do órgão transplantado e a possibilidade de retorno à terapia dialítica. Como fatores preocupantes que possam acarretar limitações na vida, bem como prejuízos na manutenção da saúde, destacaram-se a presença de efeitos colaterais das medicações imunossupressoras, a falta de orientações sobre como proceder em determinadas ocasiões e a assistência deficitária do sistema de saúde. Em virtude da ocorrência de fatores, tanto positivos, quanto negativos, implicados na vida da pessoa com o transplante renal, acredita-se que a Técnica dos Incidentes Críticos utilizada neste estudo, contribuiu para o conhecimento da sua vivência
