Choices and connections

This report, Choices and Connections, is the culmination of the Board’s work focusing on what needs to happen to improve the chances of young people with a disability participating socially and economically into the future. It published its findings under separate cover, Post-school Pathways – how ‘it is’ and how ‘it works’ for young people with disabilities. The Social Inclusion Board scoped the issues relating to the successful transition of young people with disabilities to the post-school environment. It has actively listened to what young people with a disability and their families have had to say about their experiences in working through key transition milestones. The key transition points, of moving from primary school to high school, moving from children’s services to adult services, and ultimately working through the options that might be available to them after school, are the cause of great anxiety to them all. The Board analysed all of the relevant data to quantify the client group and reviewed the current service systems and client pathways. It identified the young people with a disability that are ‘at risk’ of not making a transition from school to activities that would maximise their social and employability potential. &nbsp
