Clinical variables of preoperative risk

BACKGROUND: To identify risk variables leading to early postoperative pulmonary complication (POPC) in thoracic and upper abdominal surgery. METHODS: 297 patients submitted to elective surgery were classified as low, moderate and high risk for POPC using PORT scale, following Torrington & Henderson (1988). The patients were followed up for 72 hours postoperative. POPC were defined as atelectasis, pneumonia, tracheobronchitis, wheezing, prolonged intubation and/or prolonged mechanical ventilation. Univariate analysis was applied to study these independent variables: type of surgery, age, nutritional status (BMI), respiratory disease, smoking habit, spirometry and surgical time. Multivariate logistic regression analysis were performed in order to evaluate the relationship between independent and dependent (POPC) variables. RESULTS: POPC incidence was 12.1%. By multivariate logistic regression analysis the variables increasing chance to POPC was cough with yellow mucus (OR= 3.8), thoracic surgery (OR=2.9) compared to abdominal surgery, BMI (OR=1.13), duration of smoking (OR=1.03) and prolonged duration of surgery (OR=1.007). In the thoracic surgery group, multivariate logistic regression analysis showed: wheezing (OR=6.2), BMI (OR=1.15), long time smoking (OR= 1.04) and prolonged duration of surgery (OR= 1.007) related to POPC. CONCLUSION: The variables occurrence that increased chance to POPC in thoracic and upper abdominal surgery were: cough with yellow mucus, thoracic surgery, BMI, duration of smoking and duration of surgery. Regarding the thoracic surgery group the significance variables were wheezing, BMI, duration of smoking and duration of surgery.OBJETIVOS: O objetivo deste estudo foi identificar variáveis que facilitam o aparecimento de complicação pulmonar pós-operatória (CPP) nos pacientes submetidos às cirurgias eletivas de tórax e abdômen alto. MÉTODOS: Foram estudados 297 pacientes, avaliados e estratificados em baixo, moderado e alto risco para desenvolvimento de CPP através da escala PORT, idealizada por Torrington & Henderson ( 1988). Todos os pacientes foram acompanhados por 72 horas no pós-operatório imediato. Foram consideradas como CPP: atelectasia com repercussão clínica ou radiológica, pneumonia, traqueobronquite, broncoespasmo, intubação e/ou ventilação mecânica prolongada. Através da análise univariada, estudamos as seguintes variáveis independentes: idade, grau nutricional (índice de massa corpórea - IMC), sintomas respiratórios, doença respiratória, tabagismo, espirometria e tempo cirúrgico. Posteriormente submetemos tais variáveis à análise de regressão logística multivariada para avaliar a relação entre as variáveis independentes com a dependente e a chance de CPP. RESULTADOS: A incidência de CPP observada foi de 12,1%. Para análise estatística utilizou-se a análise univariada e posteriormente a regressão logística multivariada. Os resultados informam através da razão de chances (odss ratio-or) a participação das variáveis independentes entre si sobre a dependente (complicou/não complicou) no evento estudado. Em ambos os tipos de cirurgia, as variáveis encontradas foram: tosse com expectoração amarela (OR= 3.8); a cirurgia torácica em relação a abdominal (OR=2.9); IMC (OR=1.13); tempo de tabagismo (OR=1.03) e tempo cirúrgico (OR=1.007). Na aplicação da análise da regressão logística considerando somente a cirurgia torácica, as variáveis foram: broncoespasmo (OR= 6.2); IMC (OR= 1.15); tempo de tabagismo (OR=1.04) e tempo cirúrgico (OR=1.007). CONCLUSÕES: As variáveis de risco pré-operatórias que aumentaram a chance de CPP nas cirurgias de tórax e abdômen alto foram: tosse com expectoração amarela; cirurgia torácica, IMC; tabagismo e tempo cirúrgico. Nas cirurgias de tórax as variáveis encontradas foram: broncoespasmo, IMC, tabagismo e tempo cirúrgico.11712
