Evaluation of the relationship between serum neopterin levels and the disease's severity and mortality in pulmonary embolism

Pulmoner embolizm, hemostatik dengenin bozulduğu, morbidite ve mortalitesi yüksek olan bir hastalıktır. Bu hastalıkta erken tanı ve prognostik süreçle ilgili yeni belirteçlere ihtiyaç vardır. Bilindiği gibi inflamasyon ve hemostaz ortak aktivasyon ve geribildirim mekanizmaları içeren, birbirine bağımlı olaylardır. Son yıllarda yapılan çalışmalarda endotel mikropartiküllerinin ve trombositlerin, monositlerle yaptığı eşleşmelerin venöz tromboemboli patogenezinde rol oynadığı gösterilmiştir.Neopterin, insan monosit ve makrofajlarının interferon-? ile uyarılması sonucu salınan bir pteridin türevidir. Septik şok, viral enfeksiyonlar, tüberküloz, romatoid artrit vb inflamatuar hastalıklarda, malignitelerde, akut miyokard enfarktüsünde yüksek neopterin seviyeleri ve hastalığın progresyonu arasında ilişki bulunmuştur.Bu çalışmanın amacı pulmoner embolide monosit/makrofaj aktivasyonunu neopterin düzeyine göre değerlendirmek, neopterin seviyeleri ile hastalığın ağırlık derecesi arasındaki ilişkiyi araştırmaktır.Çalışmamıza 41 pulmoner emboli hastası ve 29 pulmoner embolisi olmayan hasta, kontrol grubu olarak dahil edilmiştir. Pulmoner emboli grubunun ortalama neopterin düzeyi 39,2 nmol/ L, kontrol grubunun ortalama neopterin düzeyi 41,93 nmol/ L saptandı. Bu değerler üst sınır (10 nmol/ L) kabul edilen değere göre yüksekti ancak pulmoner emboli ve kontrol grubu arasında neopterin düzeyleri açısından istatistiksel anlamlı fark saptanmadı. Hastalığın ağırlığı ile neopterin seviyeleri arasında korelasyon gözlenmedi.Kontrol grubunda neopterin seviyelerinin yüksek olması bu inflamasyon belirtecinin ileri yaş ve komorbiditesi olan hasta grubunda tanısal ve prognostik açıdan değerini düşürmektedir. Bu konuda neopterinin etkili bir parametre olup olmadığını anlayabilmek için daha fazla vakayı içeren çalışmalara ihtiyaç vardır.Pulmonary embolism is a disease in which hemostatic balance is damaged and that has high morbidity and mortality. New markers for early diagnosis and prognostic evaluation of the disease are needed. It?s known that, inflamation and hemostasis are linked processes that share cammon activation and feedback mechanisms. In the late years it has been shown that the coupling of endothelial microparticles and thrombocytes with monocytes is related with the pathogenesis of venous thromboembolism.Neopterin is a pteridin derivative that is released from human monocytes and macrophages by stimulation of interferon-? . High neopterin levels and correlation with the disease progression have been found in septic shock, viral infections, tuberculosis, rheumatoid artritis like inflammatory diseases, malignancies, acute myocardial infarction.The purpose of this study is to evaluate the monocyte/ macrophage activation according to neopterin levels in pulmonary embolism and to investigate the relationship between neopterin levels and the severity of the disease.Forty-one patients having, 29 patients not having pulmonary embolism were enrolled in our study. Mean neopterin level was 39,2 nmol/ L in pulmonary embolism group, 41,93 nmol/ L in control group. Although the results were higher than the upper normal limit, (10 nmol/ L) there was no statistically significant difference between the two groups. There was no correlation between severity of the disease and neopterin levels.High levels of neopterin in control group decreases the value of this marker as being a good tool for diagnosis and the prognostic evaluation of pulmonary embolism, in old patients having comorbidities. Large studies are needed to understand better if it?s an effective parameter or not in this purpose
