Thyroxine (T4) and tri-iodothyronine (T3), released excessively in the hyperthyroid patients, contribute to the dysfunction of very active endothelial cells. The role of the endothelium in the physiological state is the release and the presentation on their cells a lot of substances having a relevant meaning for the functioning of the organism. Through the release of the von Willebrandt factor (vVF), tissue plasminogen activator (tPA), plasminogen activator inhibitor-1 (PAI-1) and tissue factor pathway inhibitor (TFPI) endothelium participates in the regulation of coagulation and fibrinolytic system. The hyperthyroid patients demonstrate the dysfunction of these systems, that is the reason of higher risk of the development of thromboembolic complications. Hyperthyroidism is associated with increased activity of nitric oxide and endothelium-derived hyperpolarising factor, which have an essential meaning in modulation of vascular smooth muscle tone. The thyroid hormones also cause the increase in plasma level of endothelin-1, the vasoconstrictory peptide released from the endothelium. This, in conjunction with the increased secretion of renin from juxtaglomerular apparatus in kidneys, increased renal angiotensin-converting enzyme (ACE) activity and intensive sympathetic nervous system activity contributes to dysregulation of the blood flow and to the development of arterial hypertension in the hyperthyroid patients. Endothelial cells participate in the regulation of inflammatory and immune processes through the presentation of adhesive molecules VCAM-1 and ICAM- 1 on their surface. Thyrotoxicosis causes the increase of the levels of soluble VCAM-1 and ICAM-1, that are the competitive inhibitors of the endothelial adhesive molecules, contributing therefore to the decreased migration of lymphocytes to inflammatory places.Tyroksyna (T4) i trijodotyronina (T3), wydzielane w nadmiarze u chorych z nadczynnością tarczycy, są przyczyną dysfunkcji bardzo aktywnej warstwy śródbłonkowej układu naczyniowego (endothelium). Rolą komórek śródbłonka w warunkach fizjologicznych jest uwalnianie oraz prezentowanie na ich powierzchni wielu substancji, mających istotne znaczenie w wielu ważnych funkcjach organizmu. Poprzez uwalnianie czynnika von Willebrandta (vVF), tkankowego aktywatora plazmingenu (t-PA), inhibitora aktywatora plazminogenu-1 (PAI-1) oraz inhibitora zewnątrzpochodnego toru aktywacji (TFPI) endothelium uczestniczy w regulacji układu krzepnięcia i fibrynolizy. U chorych z nadczynnością tarczycy dochodzi do zaburzeń funkcjonowania tych układów, co jest przyczyną zwiększonego ryzyka powstania zmian zakrzepowo-zatorowych. W stanie hipertyreozy obserwuje się również zwiększoną aktywność tlenku azotu oraz śródbłonkowego czynnika hiperpolaryzującego, mających istotne znaczenie w regulacji napięcia ściany naczyń krwionośnych. Nadmiar hormonów tarczycy powoduje wzrost stężenia endoteliny-1, wazokonstrykcyjnego peptydu uwalnianego ze śródbłonka naczyniowego. To z kolei, w połączeniu ze zwiększonym uwalnianiem reniny przez komórki aparatu przykłębuszkowego nerki, wzrostem aktywności enzymu konwertującego (ACE) oraz nadmierną aktywacją układu współczulnego, przyczynia się do zaburzenia regulacji przepływu krwi oraz do rozwoju nadciśnienia tętniczego u chorych z nadczynnością tarczycy. Komórki śródbłonka naczyniowego, poprzez prezentowanie na swojej powierzchni cząstek adhezyjnych VCAM-1 oraz ICAM-1 biorą także udział w regulacji procesów zapalnych i odpornościowych. Tyreotoksykoza powoduje zwiększenie stężenia rozpuszczalnych VCAM-1 i ICAM-1, będących kompetycyjnymi inhibitorami śródbłonkowych cząstek adhezyjnych, co jest przyczyną zmniejszonej migracji limfocytów do miejsc zapalnych
