Surgical approach for retrosternal goiters

Ege Üniversitesi Tıp Fakültesi Genel Cerrahi AD'nda; Ocak 1993-Aralık 1997 tarihleri arasında öpere edilen 39 retrosternal guatrlı olgu retrospektif olarak incelendi. Olguların 23'ü kadın, 16'sı erkek iken ortalama yaş; 56 idi. Tanı yöntemi olarak kullanılan konvansiyonel akciğer grafileri ile % 61 oranında tanıya ulaşıldı. Sadece yedi olguda tanı intraoperatif olarak kondu. Olguların 36'sına subtotal tiroidektomi, 2'sine total tiroidektomi, l'ine lo-bektomi uygulandı. Sadece iki olguya sternotomi yapıldı. Sternotomi yapılan bir olguda pnömotoraks ve perikard. yaralanması gelişti. Toplam iki olguda patoloji malin geldi. Retrosternal guatrlarda cerrahi tedavi düşük morbidité ve mortalité nedeniyle tek tedavi seçeneğidir. Cerrahi girişimin öncelikle boyun insizyonu ile yapılması, gereken vakalarda sternotomi uygulanmasının güvenilir bir yöntem olduğunu düşünmekteyiz.SUMMARY Surgical Approach for Retrosternal Goiters We report 39 cases of retrosternal goiter operated at the General Surgery Department of Ege University, between January 1993'-December 1997. There were 23 female, 16 male patients and median age was 56. Diagnosis was confirmed with conventional X-ray technique in 24 patients. Only seven patients were diagnosed in the operation. 36 patients were treated with subtotal thyroidectomy, 2 were treated with total thyroidectomy and one patient with lo-bectomy. Sternotomy was performed in only two patients . In one of these cases pneumothorax and pericardial injury accured. In two cases pathologic result was malignant. The surgical approach for the retrosternal goiters is the best choice of treatment. We believe that surgery must be carried out with cervical incision and sternotomy should be performed if needed
