OBJECTIVES OF PROGRAM/INTERVENTION (NO MORE THAN THREE OBJECTIVES): To improve patient\u27s knowledge of nutrition and confidence in their ability to eat healthy. To improve access to fresh produce by 1) providing financial support, 2) introducing patients to new local Detroit farmer\u27s markets.
