Mortality trend for AMI in the elderly in Brazil (1996-2014)

Population aging increases the prevalence of ischemic diseases among the elderly. The number of deaths due to cardiovascular diseases is increasing in Brazil and are significant in the population over 60 years. Objective: To identify the trend of mortality due to acute myocardial infarction in the elderly in Brazil. Methods: Ecological study of time series. All records referring to AMI mortality in the period from 1996 to 2014 in Brazil were analyzed. Mortality rates were calculated and standardized by sex, age range by sex and region. Linear regression was performed to estimate the mortality trend. Results: The present study observed a trend of reduction in the general mortality rates due to myocardial infarction in the elderly, by sex, in the age groups from 60 to 69 years (p = 0.001) and from 70 to 79 years (p = 0.04 ), and in the South (p <0.001) and Southeast (p = 0.002) regions. On the other hand, there is a tendency to increase mortality rates for men aged 80 years or over (p = 0.001) and for the North (p <0.001) and Northeast (p <0.001) regions. Conclusion: There is a decreasing trend in the mortality rate due to acute myocardial infarction among the elderly from 60 to 79 years in Brazil, however, this fall refers to the south and southeast only. Since AMI is the main cause of death in the country, investments in preventive public policies can substantially reduce mortality among the elderly.Fundamento: O envelhecimento populacional aumenta a prevalência de doenças isquêmicas entre os idosos. O número de mortes por doenças cardiovasculares é crescente no Brasil e expressivo na população acima dos 60 anos. Objetivo: Identificar a tendência de mortalidade por infarto agudo do miocárdio em idosos no Brasil. Métodos: Estudo ecológico de séries temporais. Foram analisados todos os registros referentes à mortalidade por IAM no período de 1996 a 2014 no Brasil. As taxas de mortalidade foram calculadas e padronizadas por sexo, faixa etária por sexo e região. Foi realizada regressão linear para estimar a tendência da mortalidade. Resultados: O presente estudo observou uma tendência de redução nas taxas de mortalidade geral por infarto do miocárdio em idosos, por sexo, nas faixas etárias dos 60 até 69 anos (p= 0,001) e dos 70 até 79 anos (p=0,04), e nas regiões Sul (p < 0,001) e Sudeste (p= 0,002). Por outro lado, há uma tendência de aumento das taxas de mortalidade para os homens com 80 anos ou mais (p= 0,001) e para as regiões Norte (p < 0,001) e Nordeste p < 0,001). Conclusão: Há uma tendência decrescente na taxa de mortalidade por infarto agudo do miocárdio entre os idosos de 60 aos 79 anos no Brasil, todavia, essa queda se refere a região sul e sudeste apenas. Sendo o IAM a principal causa de morte no país, investimentos em políticas públicas preventivas podem reduzir de forma substancial a mortalidade entre os idosos
