Aging in HIV Infection : Physiopathological Mechanisms of Chronic Inflammation

Desde os primórdios da epidemia VIH-1/SIDA, foram feitos inúmeros avanços quanto ao conhecimento da sua patogénese, com criação de terapêutica antirretrovírica altamente eficaz. Dado o aumento da sobrevivência dos doentes infetados por VIH, atualmente observa-se uma mudança demográfica desta população, com um aumento gradual dos doentes com idade avançada. O envelhecimento é descrito como um processo biológico inevitável, marcado por uma perda de função de vários tecidos e órgãos, e da integridade do organismo, havendo vários potenciais mecanismos causais descritos. Nos últimos anos tem sido observado um processo de envelhecimento precoce ou acentuado nos doentes infetados por VIH, que apresentam uma esperança média de vida menor do que a população geral, uma maior mortalidade e uma maior prevalência de comorbilidades. Foram propostas várias hipóteses para explicar este processo, tais como indução de inflamação crónica, alteração da senescência imunológica e da adaptação ao stress oxidativo, fatores genéticos e epigenéticos e fatores ambientais. Este trabalho tem como objetivo descrever os mecanismos fisiopatológicos, e os seus biomarcadores respetivos, que expliquem o envelhecimento precoce que ocorre concomitantemente com a infeção por VIH-1, tanto pela ação do vírus como pela ação da terapêutica antirretrovírica, dando destaque aos que se relacionem com a indução de um estado de inflamação crónica no organismo.The infection caused by the retrovirus HIV (human immunodeficiency virus) is characterized by a drastic depletion of CD4+ T lymphocytes and subsequent loss of immune function, leading to AIDS (acquired immune deficiency syndrome). Ever since the beginning of the HIV/AIDS epidemic, numerous advances have been made regarding the knowledge of its pathogenesis and the advent of the highly effective antiretroviral therapy. Given the increase of life expectancy in general and in HIV infected patients, there has been a demographical shift giving place to a gradual rise of infected people of advanced age. Aging is a biological process marked by the loss of function in various tissues and organs and, therefore, the loss of integrity of the organism. There are currently many proposed mechanisms to explain this event. In the past years a process of early or accentuated aging has been observed in HIV infected patients. This population has a slightly reduced life expectancy when compared to the general population, a higher mortality and a higher incidence and rate of comorbidities. Many hypotheses have been proposed in order to explain this occurrence. Some examples are the persistent state of chronic inflammation, the immunesenescense, the disfunction of the adaptation to oxidative stress and genetic, epigenetic and environmental factors. This review aims to describe the pathophysiological mechanisms and their respective biomarkers that explain the premature aging that occurs alongside HIV-1 infection, both by the action of the virus and by the action of antiretroviral therapy, highlighting those that are related to the induction of a state of chronic inflammation in the organism
