Análisis de los eventos desfavorables como resultado de la atención en cirugía oral / Analysis of Unfavorable Events related to Oral Surgery Care

Background: Oral surgery is a specialty that requires theoretical preparation and surgical ability to perform the procedures. For this reason, during the learning process, students are exposed to difficulties that can cause involuntary harm to the patient. Despite compliance with established protocols, there is a possibility that unfavorable events may occur during patient care. For this reason, it is necessary to characterize and analyze these events, to offer a better quality in the health services in a Faculty of Dentistry. Purpose: To analyze the unfavorable events that occurred in the Dental School surgical area of the Pontificia Universidad Javeriana. Methods: In this descriptive observational study, all clinical records were evaluated in the period between January 1, 2014 and December 31, 2015, for the identification and subsequent analysis of the unfavorable events found. Results: Of the 1062 clinical records, 74 (7%) were identified with unfavorable events, of which 41 (56%) were established as complications, 22 (30%) were signs of unsafe care and 11 (14%) were adverse events. 9 (82%) preventable and 2 (18%) non-preventable, 100% related to exodontias. Conclusion: Adverse events in the surgical area are presented in low frequency and are related with exodontias. To mitigate these situations, it is necessary to carry out a pre-surgical planning, a meticulous elaboration of clinical history and in case of presenting some type of event, to report it.Antecedentes: La cirugía oral es una especialidad que requiere preparación teórica y habilidad quirúrgica para efectuar los procedimientos. Por este motivo, durante el proceso de aprendizaje, los estudiantes están expuestos a enfrentar dificultades que pueden provocar un daño involuntario al paciente. A pesar del cumplimiento de protocolos establecidos, existe la posibilidad de que se presenten eventos desfavorables durante la atención de los pacientes. Por este motivo, es necesario caracterizar y analizar dichos eventos, para ofrecer una mejor calidad en los servicios de salud en una Facultad de Odontología. Objetivo: Analizar los eventos desfavorables que se presentaron en el área quirúrgica de la Facultad de Odontología de la Pontificia Universidad Javeriana. Métodos: En este estudio observacional descriptivo se evaluaron todas las historias clínicas en el período comprendido entre el 1 de enero de 2014 y el 31 de diciembre de 2015, para identificar y posteriormente analizar los eventos desfavorables registrados. Resultados: De 1062 historias clínicas, se identificaron 74 (7 %) con registro de eventos desfavorables, 41 (56 %) de las cuales se tipificaron como complicaciones, 22 (30 %) como indicios de atención insegura y 11 (14 %) como eventos adversos, 9 (82 %) prevenibles y 2 (18 %) no prevenibles relacionados con exodoncias. Conclusiones: Los eventos adversos en el área quirúrgica se presentan en baja frecuencia y se relacionan en su totalidad con exodoncias. Para mitigar estas situaciones es preciso realizar una planificación prequirúrgica, una meticulosa elaboración de historia clínica y, en caso de presentarse algún tipo de evento, reportarlo
