Coronary artery disease continues to be a major cause of death. In the developed countries, mortality after a myocardial infarction is directly related to the extent of myocardial necrosis. Reperfusion of the ischemic myocardium (spontaneous or after medical or surgical treatment) eliminates infarct size and decreases mortality markedly. Reperfusion of the ischemic myocardium salvages reversibly injured cells, but it can also accelerate destruction of irreversibly injured cells or mildly injured ones.
