Πειραματικό έμφραγμα του μυοκαρδίου: επαναιμάτωση υπό διάφορα επίπεδα αρτηριακής πίεσης

Coronary artery disease continues to be a major cause of death. In the developed countries, mortality after a myocardial infarction is directly related to the extent of myocardial necrosis. Reperfusion of the ischemic myocardium (spontaneous or after medical or surgical treatment) eliminates infarct size and decreases mortality markedly. Reperfusion of the ischemic myocardium salvages reversibly injured cells, but it can also accelerate destruction of irreversibly injured cells or mildly injured ones. The purpose of this study was to evaluate the possible effect of the decreased arterial pressure during reperfusion on the infarct size. Nineteen experimental animals (pigs) were randomized to two groups. At the beginning, animals of both groups were subjected to thoracotomy and mid sternotomy and then to proximal left anterior descending coronary artery occlusion, for one hour. Finally, reperfusion was achieved by release of the ligation for two hours. During reperfusion, in group I (n=7), mean arterial pressure (MAP), was kept between 80-95mmHg by administering crystalloid fluids and nitroglycerin (NTG) in therapeutic dose while in group II (n=12) MAP was kept between 60-75mmHg by administering nitroglycerin (NTG) in appropriate doses. MAP, left atrial pressure (LAP), right atrial pressure (RAP), heart rate (HR) and distal coronary pressure (DCP) were recorded during ischemia and reperfusion. In addition, double product (DP) was calculated. During control and ischemic periods, there were no significant differences in the hemodynamic variables between the two groups. However, during reperfusion, MAP (86,7±6,2mmHg in group I versus 68,5±5,5mmHg in group II, p=0,000) and DP (12346±1594 in group I versus 10419±1610 in group II, p=0,022), were significantly higher in group I. No significant differences were observed between groups in the other hemodynamic variables. The myocardium at risk was similar in both groups. The infarcted myocardium (IM) expressed as the percentage of the myocardium at risk (MR) and as percentage of the whole mass of the left ventricle (MLV) was significantly smaller in group I with relatively high arterial pressure than in group II with relatively low arterial pressure (IM=50,2±4,3% of MR in group I versus 68,7±7,0%in group II, p=0,000 and IM=13,5±5,7% of MLV in group I versus 19,5±5,0% in group II, p=0,031). In conclusion, the data of the present experimental study suggests that decreased arterial pressure during reperfusion expands myocardial infarct size.Σκοπός της παρούσας διατριβής ήταν να μελετηθεί η επίδραση της ελαττωμένης αρτηριακής πίεσης κατά την επαναιμάτωση στην έκταση του εμφράγματος του μυοκαρδίου σε χοίρειο πειραματικό πρότυπο. Χρησιμοποιήθηκαν 19 χοίροι (βάρους 20-27 kg) που τυχαιοποιήθηκαν σε 2 ομάδες. Σ’ όλα τα πειραματόζωα μετά τη μέση στερνοτομή και διάνοιξη του περικαρδίου προκαλείτο ισχαιμία με απολίνωση της πρόσθιας κατιούσας στεφανιαίας αρτηρίας για μια ώρα και επαναιμάτωση μ άρση της απολίνωσης για δύο ώρες. Κατά τη διάρκεια της επαναιμάτωσης η μέση αρτηριακή πίεση (ΜΑΠ) διατηρήθηκε σε επίπεδα 80-95 mmHg στην ομάδα Ι με χορήγηση κρυσταλλοειδών διαλυμάτων και νιτρογλυκερίνης σε θεραπευτική δόση, ενώ στην ομάδα ΙΙ προκλήθηκε πτώση της ΜΑΠ σε επίπεδα 60-75mmHg με χορήγηση νιτρογλυκερίνης στην απαιτούμενη προς τούτο δόση. Κατά τη διάρκεια της επαναιμάτωσης η ΜΑΠ και το διπλό γινόμενο ήταν μεγαλύτερα σε βαθμό στατιστικά σημαντικό στην ομάδα Ι απ’ ότι στην ομάδα ΙΙ. Τα δεδομένα της παρούσας πειραματικής μελέτης δείχνουν ότι η μείωση της αρτηριακής πίεσης κατά την επαναιμάτωση δυνατόν να αυξήσει την έκταση του εμφράγματος του μυοκαρδίου
