The effect of short bowel syndrome, with or without myenteric plexus destruction in liver morphology and fruction

The application of benzalkonium chloride solution (BAC) to the serosal surface of the intestinecauses intrinsic denervation of the myenteric plexus, changes intestinal morphologyand slows down the intestinal passage.Intrinsic denervation of the myenteric plexus, in a rat model of short bowel syndrome , asa technique of improvement of this condition.Twenty four male Wistar rats ,with mean body weight 31 l±20g , underwent 80% midsmallbowel resection with end to end anastomosis and wedge biopsy of the liver. In 12 ofthem , BAC solution 0.1% was applied to a 2 cm segment of jejunum according to the modifiedFox method.The changes in intestinal and hepatic morhology, liver function tests wererecorded, one month after the initial intervention.In the denervated segment of the BAC treated group (Group 1), a statistically significantincrease (p<0.05) was noted in villous height by 33.2% and crypt depth by 26.4% respectively,compared to the control group.There was also an increase of 26% in muscle thicknessand 109.6% in intestinal diameter (p<0.05). No statistically significant difference was observedin the untreated jejunal segments and ileum, between the two groups.The mean bodyweght in group 1 was significantly higher (p=0.003) compared to that of group 2, one monthafter the initial operation( an increase of 15% compared to the control group 2).The values of liver function tests, protein and albumin didn't differ between the twogroups. Steatosis of the liver wasn't observed in any of rats, in groups 1 and 2. Extensiveliterature review helped us interpret the steatosis that is observed in extensive intestinal bypass,but not in the corresponding intestinal resection and that can be attributed, to a situationof continued protein malnutrition.Chemical denervation of a jejunal segment with BAC, in short bowel syndrome is safeand effective method , that can also be applied during the initial intestinal resection, withoutthe need for additional anastomoses. Continuation of research in superior mammals is proposed,so that the described method can henceforth find application in the clinical practice.Η εφαρμογή διαλύματος benzalkonium chloride (BAC) στον ορογόνο τμήματος του λεπτούεχ'τέρου προκαλεί καταστροφή του μυεντερικού πλέγματος, αλλάζει την εντερική μορφολογίακαι επιβραδύνει την εντερική δίοδο.Η καταστροφή του μυεντερικού πλέγματος σε πειραματικό μοντέλο αρουραίων με συνδρόμουβραχέος εντέρου, ως μία τεχνική βελτίωσης αυτού.Σε 24 αρσενικούς αρουραίους Wistar μέσου σωματικού βάρους 311±20g πραγματοποιήθηκεεκτεταμένη εκτομή του 80% της μεσότητας του λεπτού εντέρου, αποκατάστασητης εντερικής συνέχειας με τελικο-τελική αναστόμωση και σφηνοειδή βιοψία ήπατος.
