Establishment of a multi-center microcephaly database: pathogenesis and phenotype of children with microcephaly at the Department for Neuropediatrics in Berlin 2000-2010

Mikrozephalie (MC) bezeichnet einen Kopfumfang, der mehr als zwei Standardabweichungen unterhalb des entsprechenden Durchschnitts liegt. Die Genese der MC ist heterogen, der Phänotyp variabel und das Spektrum assoziierter Erkrankungen groß. Ziel der Arbeit ist die Erstellung einer Datenbank zur Erfassung (i) der angenommenen Pathogenese der MC, (ii) der Prävalenz der ZNS Struktur-Veränderungen, (iii) des Phänotypspektrums kindlicher MC und assoziierter Erkrankungen und (iv) des diagnostischen Vorgehens. Methoden: Im Rahmen einer Pilotstudie wurden zunächst retrospektiv die Krankengeschichten von 474 Kindern (64% Jungen, 36% Mädchen), welche eine Mikrozephalie aufweisen (Kopfumfang < 3\. Perzentile) und im Zeitraum von 2000 bis 2010 in der Neuropädiatrie der Charité behandelt wurden, ausgewertet und die Daten anonymisiert erfasst. Ergebnisse: In unserer Kohorte hatten 13% (n=60) eine primäre (kongenitale) und 18% (n=88) eine sekundäre (postnatale) MC; bei den restlichen Patienten war auf Grund der Datenlage keine eindeutige Zuordnung möglich. Bei 48% (n=229) handelte es sich um eine proportionierte und bei 51% (n=243) um eine disproportionierte MC. Ursächlich waren bei 15% (n=70) chromosomale/genomische Aberrationen sowie monogene Erkrankungen, bei 3% (n=14) Stoffwechseldefekte, bei 2% (n=11) Kraniosynostosen, bei 25% (n=117) perinatale und bei 2% (n=10) postnatale Hirnschäden. Eine genetische Ursache wurde bei 14% (n=67) auf Grund von Konsanguinität und/oder weiterer Betroffener vermutet. Bei 42% (n=199) blieb die Ursache der MC unklar. 74% (n=355) der Kinder erhielten im Durchschnittsalter von 14 Monaten eine MRT des Schädels (48% (n=227) auffälliger Befund, 26% (n=128) Normalbefund). Die häufigsten assoziierten morphologische ZNS Auffälligkeiten der weißen Substanz bei 12% (n=57) und Gyrierungsstörungen bei 5% (n=25). 65% (n=310) der Kinder hatten eine Entwicklungsstörung, 45% (n=212) hatten eine Epilepsie, zusätzlich waren kraniofaziale Dysmorphien, ophthalmologische Befunde und außerhalb des ZNS assoziierte Erkrankungen auffällig. Schlussfolgerungen: Ursachen und der Phänotyp kindlicher MC sind heterogen, und der hohe Prozentsatz der Kinder mit MC unklarer Genese weist auf weitere Heterogenität hin. Auf Basis der empirischen Datenerfassung soll der diagnostische Algorithmus, insbesondere bei Kindern mit einer primären Mikrozephalie, optimiert werden.Microcephaly (MC) is defined as a head circumference more than 2 standard deviations (SD) below the mean for age, gender and ethnicity. The pathogenesis of MC is heterogeneous, the phenotype variable and the spectrum of associated disorders is large. Aim of this study was to generate a data bank comprising (i) the putative pathogenesis oft the MC, (ii) the prevalence of structural brain abnormalities, (iii) the phenotype of MC and associated disorders and (iv) the diagnostic steps. Methods: In this pilot study, the hospital charts of 473 children (64% boys, 36% girls) with a microcephaly (head circumference < 3\. percentile) and treated in the Dept. Neuropediatrics of the Charité between 2000 and 2010 were reviewed, and the data collected in an anonymous form. Results: In our cohort, 13% (n=60) had a primary (congenital) and 18% (n=88) a secondary (postnatal) MC; such a classification was not possible in the other patients due a lack of sufficient data. Head circumference and other anthropometric data were proportionate in 48% (n=229) and disproportionale in 51% (n=243) of MC patients. Etiology of the MC was genetic in 15% (n=70), and and the phenotype is highly suspicious of a genetic disorder in 14% (n=67). MC causes further included metabolic diseases In 3% (n=14), craniosynostosis in 2% (n=11) and perinatally brain injury in 25% (n=117) or postnatal brain injury in 2% (n=10). In 42% (n=199) the pathogenesis remained unclear. 74% (n=355) of the children had a cMRI at a mean age of 14 months, in 48% (n=227) the results were abnormal and in 26% (n=128) MR revealed normal images of the brain. The most frequent structural brain lesions were: White substance anomalies in 12% (n=57) and Gyration anomialies in 5% (n=25) 65% (n=310) of the children had a developmental disability, 45% (n=212) had epilepsy, furthermore craniofacial dysmorphism, ophthalmologic and audiologic disorders and extra CNS organ involvement. Conclusion: Pathogenesis and phenotype of MC is heterogeneous, and the high percentage of MC of unknown etiology reveals further heterogeneity. On the basis of the empirical data collection, the diagnostic algorithm, especially for children with primary MC, should be optimized
