Uso de opioides en el tratamiento del dolor

An enigma in the management of pain is its etiology, it was generated to the extent that became available op- erational techniques and diagnostic imaging, factors to which was added the lack of comprehensive patient man- agement, which overlapped the diagnosis, pain relief, thereby generating erroneous behavior and postponing a fundamental right of the patient, to be free from pain, it probably was generated since 1921 when Sir Zachary Cope, in his book “Early Diagnosis of Acute Abdomen”, proscribed the use of analgesia –primarily morphine– in patients with abdominal pain, because achieving this purpose, signs and symptoms of an abdominal catastrophe in progress were obscured. Until now, this approach in the management of acute pain is a barrier to achieve qual- ity standards in the management of patients with pain, especially acute pain.Un enigma en el manejo del dolor, es su etiología, ello se generó en la medida en que se fue disponiendo de técnicas operativas y de imágenes diagnósticas, factores a los que se adicionó la falta de tratamiento integral del paciente, que sobreponía el diagnóstico al alivio del dolor, generando con ello conductas erróneas y que pospo- nían un derecho fundamental del enfermo, a no padecer dolor; ello probablemente se generó desde el año 1921 cuando Sir Zachary Cope, en su obra Diagnóstico Temprano del Abdomen Agudo, proscribía el uso de analgesia –morfina fundamentalmente– en pacientes con dolor abdominal, porque al lograr este propósito se oscurecían los signos y síntomas de una catástrofe abdominal en curso. Hasta hoy esa conducta en el manejo del dolor agudo sigue como barrera para alcanzar estándares de calidad en el manejo de los pacientes con dolor, y muy especial- mente dolor agudo
