STAT3 protects from liver injury and fibrosis during inflammatory liver damage

Exzessiver, langjähriger Alkoholkonsum, virale Leberinfektionen und Cholestase sind die Hauptursachen von chronischen Leberschäden und deren Folgeerscheinungen, Leberfibrose, sowie in fortgeschrittener Form, Leberzirrhose. Leberzirrhose erhöht nicht nur die Sterblichkeit der Patienten sondern auch das Risiko an Leberkrebs zu erkranken. Mausmodelle, die schwere humane Erkrankungen studierbar machen, sind weltweit von höchster medizinischer Relevanz um erfolgversprechende therapeutische Ansätze für die Patienten zu entwickeln. Für unsere Studie verwendeten wir Mäuse, die nicht in der Lage sind, die kanalikuläre Phospholipid Flippase Mdr2 zu exprimieren. Als Folge davon produzieren diese Mäuse eine toxische Galle, die schlussendlich zu einer chronisch entzündeten Leber (sklerosierende Cholangitis) führt. In dieser Studie zeigen wir, dass das Protein Signal Transducer and Activator of Transcription 3 (STAT3) die Leber während chronisch entzündlichen Prozessen schützt, da es eine Vielzahl von protektiven zellulären Mechanismen kontrolliert. Abwesenheit von STAT3 in parenchymatischen Leberzellen (Hepatozyten) und Gallengangsepithelzellen (Cholangiozyten) von mdr2-/- Mäusen führt zu dramatischen Leberschäden und einer reduzierten Regenerationsfähigkeit um neue, gesunde Hepatozyten zu bilden. Folglich kommt es zur verstärkten Ablagerung von Kollagenen - einem charakteristischen Prozess der auch während der Entwicklung der Leberfibrose beim Menschen auftritt. Biochemische und molekularbiologische Untersuchungen zeigen, dass STAT3 sowohl Hepatozyten als auch Cholangiozyten bei entzündlichem Leberschaden schützt.Chronic alcohol consumption, viral hepatitis infections and cholestasis are major risk factors for the development of liver fibrosis and it's end-stage disease, cirrhosis. Moreover, cirrhotic livers remain a major risk factor for the development of hepatocellular carcinoma (HCC) which is one of the most common cancers with very poor prognosis. Mouse models that develop chronic liver damage and resemble human liver fibrosis are urgently needed to study the role of potently beneficial drugs that are developed in order to treat patients. These mouse models could contribute to a better understanding of the complex processes that lead to fibrotic remodelling of the liver. Here we demonstrate that the protein Signal Transducer and Activator of Transcription 3 (STAT3), a master regulator of hepatocyte survival mediates a plethora of cellular functions that lead to protection from fibrosis in the chronically inflammed liver. We used mice lacking the multidrug resistance gene 2 (mdr2-/-) as a model for inflammatory liver damage. We demonstrate that conditional inactivation of STAT3 in hepatocytes and cholangiocytes (stat3hc) of mdr2-/- mice strongly aggravated liver injury, hepatocyte necrosis and increased deposition of collagen I and collagen III by non-parenchymal cells which eventually led to fulminant liver fibrosis. Biochemical and molecular characterization suggested that STAT3 exerts hepatoprotective functions in both, hepatocytes and cholangiocytes. Loss of STAT3 in hepatocytes led to upregulation of bile acid biosynthesis genes and downregulation of hepatoprotective epidermal growth factor receptor and insulin-like growth factor 1 signaling pathways.
