Acupuncture et version fœtale : identification des éléments influençant le taux de version fœtale par acupuncture

Introduction: the breech is a type of normal delivery involving risks which represents 3 to 4% of presentations of the fetus. A delivery happening with such presentation would lead to a fetal-maternal risk. In order to prevent morbidity to patients, the National College of Gynecology and Obstetrician of France (CNGOF) recommends external cephalic version.However, this method of manipulation may imply risks and alternatives exist to achieve fetal version. This paper will present acupuncture stimulation of the 67v point.Aims: Identify the components of such a practice that are likely to explain the success rate of a fetal version realized by the use of acupuncture.Methods : The first phase of the research consisted in doing a review of the academic literature in that topic. Data-bases used during the research were provided by PubMed, Em-Consult, GERA and AGORE. Keywords were chosen using the Thesaurus Medline. The second phase consisted in refining the extensive body of documents to a limited selection.Results: a set of sixteen articles were finally chosen to explain the components in the practice of acupuncture that can explain the success rate of a fetal version using that method. Those components are : the earliness of the treatment, multi-parity, the frequency of the stimulation oneach feet for ten minutes every day, the complete breech, the asiatic background of the method.Conclusion: Some of the components of this method working in favour of a fetal version using acupuncture could be verified. The whole set of information collected enabled the proposition of using acupuncture for the fetal version in order to optimize results.Introduction : Le siège est une présentation eutocique à risque représentant 3 à 4% des présentations fœtales. Un accouchement lors d’une telle présentation induit un risque fœto-maternel. Dans le but de prévenir la morbidité des patients, le Collège National de Gynécologie et Obstétricien de France (CNGOF) préconise une version par manœuvre externe. Or, cette prise en charge n’est pas dénuée de risque. Parallèlement, des alternatives existent afin d’obtenir une version fœtale. Celle étudiée ici est l’acupuncture par la stimulation du point 67v.Objectifs : Identifier les éléments susceptibles d’expliquer le taux de réussite de la version fœtale par acupuncture.Matériels et méthode : une revue de la littérature a été effectuée. Les bases de données utilisées sont ceux de PubMed, Em-consult, GERA et AGORE. Les mots clés ont été choisis à l’aide du thesaurus Medline. Une étape de sélection a dû être effectuée suite à la quantité d’articles recueillis.Résultats : seize articles ont été sélectionnées. Les éléments identifiés susceptibles d’expliquer la version fœtale par acupuncture sont : la précocité du traitement, la multiparité, le nombre d’une stimulation par jour pendant dix minutes sur chaque pied, le siège complet, l’origine asiatique.Conclusion : certains éléments permettant de favoriser la version fœtale par acupuncture ont pus être identifiés. L’ensemble des informations recueillis ont permis la proposition d’une prise en charge de la version fœtale par acupuncture dans le but d’optimiser les résultats
