Neoplastic transformation of verrucous lichen planus has rarely been described. The occurrence of verrucous and cuniculatum carcinoma successively is exceptional. We report the first case in our knowledge. A 60-year-old woman, presented since 2004 pruritic, erythematosquamous and verruccous plaques with scleroatrophic surface by places on legs, hips, and sacral area. She presented anonychia of toes with acromic plaques on vulva. She developed in July 2010 plantar masse on 1 verrucous plaque. Tumor biopsy revealed verrucous carcinoma and biopsy of plaques confirmed a diagnosis of verrucous lichen planus
