Prevalência e aspectos clínicos e epidemiológicos de mulheres HIV positivas infectadas pelo Papilomavírus humano (HPV) genital na Amazônia Brasileira

The Human Immunodeficiency Virus (HIV) has been shown to potentiate the oncogenic mechanisms of Genital Infection by the human papillomavirus (HPV) , it is believed that the state of immunocompetence of the host may play a role in the evolution of cervical lesions . HPV infects a large proportion of women worldwide , with an STD (sexually transmitted disease) more common in the world with approximately 291 million carriers of this virus women. This keeps information related to the annual incidence of nearly 500,000 cases of cervical cancer . According to the National Cancer Institute (INCA) , this is the second most frequent cancer in a Brazilian population and more common in Pará The objective is to detect the prevalence and clinical and epidemiological aspects of the HIV -infected women postivas HPV. It is cross - analytical study . The samples are from patients treated at the Counseling and Testing Center (ATC) of the municipality of Tucuruí from March 2011 to July 2013 for detection and enumeration of CD4 and CD8 T cells was performed the technique of nested- PCR , and qPCR , and the slides were stained by the Papanicolaou method. The prevalence of HPV was 68 % . Most were married / common-law marriage (63.5 %) , illiterate / incomplete primary (50 % ). Regarding gynecological history, 94.59 % used condoms in all relationships , the median age of first sexual intercourse was 13 years , and only 24.3 % had their first intercourse at age 14 or less . Among the respondents, 82.4 % had 2 or more sexual partners during their lifetime , with a median of 6 partners . HIV infection is considered a risk factor for development of cervical cancer and during the survey data showed that the CD4 + count is inversely proportional to the emergence of this disease were correlated . Eighty percent of the samples that were positive for HPV DNA were detectable after 9 subtypes (6, 11, 16 , 18, 31 , 33, 35 , 52, 58) studied, and 88% positive DNA HPV of the samples were subtyped with one or more subtypes high risk for cervical cancer . O HPV infecta uma grande parcela da população feminina mundial, sendo a DST (Doença Sexualmente Transmissivel) mais comum no mundo com aproximadamente 291 milhões das mulheres portadoras desse vírus. Essa informação mantém relação com a incidência anual de quase 500 mil casos de câncer de colo do útero. De acordo com o Instituto Nacional do Câncer (INCA), este é o segundo câncer mais incidente na população brasileira, e o mais incidente no Pará. O objetivo do estudo é de detectar a prevalência e os aspectos clínicos e epidemiológicos nas mulheres HIV postivas infectadas pelo HPV. Trata-se de estudo transversal-analítico. As amostras são de pacientes atendidas no Centro de Testagem e Aconselhamento (CTA) do município de Tucuruí no período de março de 2011 a julho de 2013,para detecção e contagem de células T CD4 e CD8 foi realizado a técnica de nested-PCR, e qPCR, e as lâminas foram coradas pelo método de Papanicolaou. A prevalência encontrada do HPV foi de 68%. A maior parte era casada/união estável (63,5%), analfabetas/fundamental incompleto (50%). Em relação aos antecedentes ginecológicos, 94,59% utilizam camisinha em todas as relações, a mediana de idade da coitarca foi de 13 anos, sendo que apenas 24,3% tiveram a primeira relação com 14 anos ou menos. Dentre as entrevistadas, 82,4% tiveram 2 ou mais parceiros sexuais durante a vida, com mediana de 6 parceiros. A infecção por HIV é considerada um fator de risco para o desenvolvimento do câncer de colo uterino e durante a pesquisa foram correlacionados dados que demonstravam a contagem de CD4+ ser inversamente proporcional ao surgimento dessa enfermidade. Oitenta por cento das amostras que estavam positivas para o DNA HPV foram detectáveis aos 9 subtipos (6, 11, 16, 18, 31, 33, 35, 52, 58) estudados, e 88% das amostras positivas para o DNA HPV foram subtipadas com 1 ou mais subtipos de alto risco para o câncer do colo uterino. A avaliação da população de mulheres soropositivas em relação à presença de HPV e seus subtipos foram de grande importância para auxiliar no diagnóstico precoce e tratamento, fornecem subsídios para programas regionalizados de prevenção e manejo dessas infecções
