Study of the interaction of obesity with female hormones in a murine model of mixed asthma.

Um dos fenótipos da asma é a asma mista no qual se observa aumento do número de neutrófilos, eosinófilos e potencial resistência aos efeitos anti-inflamatórios dos glicocorticoides. Considerando que mulheres obesas e na pós-menopausa parecem ser mais sensíveis ao desenvolvimento deste fenótipo, neste estudo avaliamos o papel da obesidade e dos hormônios sexuais femininos em modelo murino de asma mista. Para tanto, fêmeas de camundongos Balb/c (21 dias) foram submetidas à dieta hiperlipídica (DH) ou convencional (DC). Após 10 semanas de dieta, os animais do grupo DH e do grupo DC foram sensibilizados (Ovoalbumina, OVA + Adjuvante completo de Freund, s.c. Dia 0). Os animais foram desafiados com OVA dissolvida em PBS (i.n.) nos dias 21, 22 e 23. Os estudos foram conduzidos 24h após o último desafio (Grupo: 1 ciclo de desafio). Como controle foram utilizados animais não sensibilizados submetidos à instilação nasal de PBS. A interação da obesidade com os hormônios sexuais femininos (HSF) neste modelo foi estudada em grupos de animais submetidos à asma mista que tiveram seus ovários removidos cirurgicamente (OVx) 7 dias após o último desafio. Após 10 dias da OVx os animais foram reexpostos ao desafio com OVA nos dias 40, 41 e 42 (Grupo: 2 ciclos de desafios). Como controle foram utilizados animais re-desafiados, falsamente OVx (Sham-OVx). Em uma série paralela de estudos, animais do grupo DH com 1 ciclo de desafio foram tratados com budesonida (100mg/kg, i.n.) 1h antes de cada desafio. Os resultados gerados indicaram que a exposição dos animais à dieta DH aumentou o tecido adiposo abdominal e gonadal, massa corporal e a concentração sérica de colesterol, glicemia, leptina, IL-6, e MCP-1, sugerindo a presença de obesidade e inflamação sistêmica. Animais OVx apresentaram redução do peso do útero, dos níveis de estradiol e ciclo estral compatível ao diestro. Após a indução da asma mista, a inflamação pulmonar foi determinada pela quantificação das células recuperadas no lavado broncoalveolar (LBA) e mecânica respiratória avaliada pela resistência das vias aéreas (Rn), pulmonar (G) e da elastância (H) frente à metacolina (MCh) utilizando o equipamento FlexiVent. O número de células recuperadas no LBA dos animais do grupo DH e DC com 1 ciclo de desafio elevou significativamente em relação aos respectivos grupos controle, todavia não diferiu entre os grupos (DH versus DC). O LBA do grupo DH e DC se caracterizou por elevado número de neutrófilos (33.76±3.15*x104cels/ml) e eosinófilos (20.19±2.35* x104cels/ml). Além disto, animais do grupo DH e DC com 1 ciclo de desafio desenvolveram similar aumento de Rn e G quando comparados aos seus controles. O perfil inflamatório pulmonar do LBA, a Rn e G obtidos nos animais mantidos com DH após os desafios (grupo 1) não foram alterados pelo tratamento dos animais com budesonida. Por outro lado, a budesonida foi efetiva em reduzir apenas a concentração no soro de IgG3 sem afetar, contudo, a elevada concentração sérica de IgE e IgG (1, 2b) e de IL-5. A magnitude da inflamação pulmonar causada pela reexposição à OVA dos animais DH e DC com asma mista não diferiu entre os grupos OVx e Sham-OVx. Similarmente, o aumento de Rn e de G observado pela reexposição à OVA nesses animais não foi alterado pela OVx. Todavia, a reexposição à OVA de animais OVx em animais mantidos sob a DH induziu Rn e G de maior magnitude quando comparada a de animais OVx mantidos sob a DC. Sistemicamente animais do grupo DH quando reexpostos à OVA apresentaram aumento da concentração de GMCSF, IL-1b e redução dos valores de IgG. Concluindo, os dados obtidos com o modelo de asma mista podem sugerir que na vigência de baixas concentrações de estradiol, animais obesos quando reexpostos ao agente sensibilizante se tornam mais sensíveis à Mch do que indivíduos não obesos nas mesmas condições de maneira exacerbar a resistência das vias aéreas e pulmonar. Ainda, os resultados apresentados podem, eventualmente, contribuir para a melhor compreensão da piora dos sintomas da asma mista entre mulheres obesas e na pós-menopausa.One of the asthma phenotypes is mixed asthma in which there is an increase in the number of neutrophils, eosinophils and potential resistance to the anti-inflammatory effects of glucocorticoids. Considering that obese and postmenopausal women seem to be more sensitive to the development of this phenotype, in this study we evaluated the role of obesity and female sex hormones in a murine model of mixed asthma. For that, females of Balb/C mice (21 days) were submitted to a high-fat (DH) or conventional (DC) diet. After 10 weeks of diet, the animals in the DH group and the DC group were sensitized (Ovoalbumin OVA + Freund\'s complete adjuvant, s.c. Day 0). The animals were challenged with OVA dissolved in PBS (i.n.) on days 21, 22 and 23. The studies were conducted 24 hours after the last challenge (Group: 1 challenge cycle). As control, non-sensitized animals subjected to nasal PBS instillation were used. The interaction of obesity with female sex hormones (HSF) in this model was studied in groups of animals submitted to mixed asthma that had their ovaries surgically removed (OVx) 7 days after the last challenge. After 10 days of OVx, the animals were reexposed to the challenge with OVA on days 40, 41 and 42 (Group: 2 challenge cycles). As control, re-challenged animals, falsely OVx (Sham-OVx) were used. In a parallel series of studies, animals in the DH group with 1 challenge cycle were treated with budesonide (100mg/kg, i.n.) 1h before each challenge. The results generated indicated that the animals\' exposure to the DH diet increased abdominal and gonadal adipose tissue, body mass and the serum concentration of cholesterol, glycemia, leptin, IL-6, and MCP-1, suggesting the presence of obesity and systemic inflammation. OVx animals showed a reduction in uterine weight, estradiol levels and estrus cycle compatible with diestrus. After induction of mixed asthma, pulmonary inflammation was determined by quantifying the cells recovered from bronchoalveolar lavage (LBA) and respiratory mechanics assessed by airway (Rn), pulmonary (G) and elastance (H) resistance against methacholine (MCh) using the FlexiVent equipment. The number of cells recovered in the LBA of animals in the DH and DC group with 1 challenge cycle increased significantly in relation to the respective control groups, however it did not differ between groups (DH versus DC). The LBA of the DH and DC group was characterized by a high number of neutrophils (33.76±3.15*x104 cells/ml) and eosinophils (20.19±2.35* x104 cells/ml). In addition, animals in the DH and DC group with 1 challenge cycle developed a similar increase in Rn and G when compared to their controls. The pulmonary inflammatory profile of LBA, Rn and G obtained in animals maintained with DH after the challenges (group 1) were not altered by treatment of the animals with budesonide. On the other hand, budesonide was effective in reducing only the serum concentration of IgG3 without affecting, however, the high serum concentration of IgE and IgG (1, 2b) and IL-5. The magnitude of pulmonary inflammation caused by reexposure to OVA of DH and DC animals with mixed asthma did not differ between the OVx and Sham-OVx groups. Similarly, the increase in Rn and G observed by reexposure to OVA in these animals was not altered by OVx. However, the reexposure to OVA of OVx animals in animals kept under DH induced Rn and G of greater magnitude when compared to that of OVx animals kept under DC. Systemically animals of the DH group when reexposed to OVA showed an increase in the concentration of GMCSF, IL-1b and a reduction in IgG values. In conclusion, the data obtained with the mixed asthma model may suggest that in the presence of low concentrations of estradiol, obese animals when reexposed to the sensitizing agent become more sensitive to Mch than non-obese individuals in the same conditions in order to exacerbate the resistance of the pathways air and lung. Still, the results presented may, eventually, contribute to a better understanding of the worsening of the symptoms of mixed asthma among obese and postmenopausal women
